Question: Are there certain hormones that are released with positional changes? Such as standing and lying down?

And can pressure in the abdomen release hormones such as adrenaline?

Vasopressin

Epinephrine

Aldosterone

2 Applying pressure on the abdomen typically does not cause release of adrenaline. However if the pressure is severe enough to cause pain, it can do result in release of adrenaline

What conditions could cause the shortness of breath that were related to positional or random episodes?

I will give two examples related to hormones, because the causes of shortness of breath are vast.

1 In a medical condition called Primary Aldosteronism, there is excess Aldosterone ( a hormone) in the blood which can lead to high blood pressure. Uncontrolled and untreated blood pressure can cause shortness of breath. But this is usually unrelated to posture.

2 In a medical condition called Pheochromocytoma, there is excess 'adrenaline' in the blood. This can lead to shortness of breath.
In this problem, there can be significant fluctuation in blood pressure when changes in posture occur.

I am supposed to do a 24 hour urine collection for a carcinoid because symptoms happen at random as well. I can get flushed, have tremors, tachycardic, have to take deep breaths, become irritable/edgy, feel like I am going crazy. Blood pressure does not fluctuate much. Sometimes it feels like a rush of adrenaline.Then goes away within 30 minutes or so. I had slightly high levels of plasma metanephrines but then another test was normal. CT scan of abdomen did not show any abnormalities. But I don't understand how positions, lifting, bending could effect this. And why would laying flat help me? I have seen a psychiatrist several times and they do not believe it is related to psychiatric problems.

The 24 hour urine test for serotonin helps diagnose carcinoid. You may wish to request your doctor to consider adding fractionated catecholamines and metanephrines to the 24 hour urine sample along with creatinine. These look for pheochromocytoma. I see your blood metanephrines were mildly elevated, and the 24 hour collection for the mentioned test is to confirm the problem.
Sorry to learn about your bothersome symptoms. When I see someone like you in my practice, I typically order the following blood tests in addition to a detailed physical examination:

CBC (Complete Blood Count, also known as Hemogram; includes Hemoglobin, WBC and Platelet counts)
Electrolytes (Sodium and Potassium in particular)
HbA1c (Glycosylated Hemoglobin = your last 3 months' glucose average). Also known by other names such as GlycoHemoglobin or Glycated Hemoglobin or A1c
Liver function tests (SGOT , SGPT, Albumin, Bilirubin, Alkaline Phosphatase)
Kidney function tests (BUN, Creatinine)
TSH (checks your thyroid)
25 hydroxy Vitamin D levels (ideal range 40 to 60 ng/ml = 100 to 150 nmol/liter)

None of these tests require any fasting and can be done at any time of the day Correct diagnosis and treatment requires the opportunity to examine the patient so you must see an endocrinologist in-person.
