Back Ground:- My Mother In Law Have A XXXXXXX Tumor

Question: Back ground:- My Mother in Law have a XXXXXXX Tumor Detected (Meningioma) Detected in MRI, DOB 07/01/62, Size is 22 AP X23(TR)x24 CC mm, Doctor advised that he will do a shunt operation first to remove the extra brain fluid which will help the patient in current issues patient is having (Headachace,Not able to walk properly with one leg etc.) and then he will perform a Cyber Knife treatment to get rid of Tumor.

Question:- 1) Can cyber Knife be done first ? With which we can suppress the tumor and then see if that open up some way for brain Fluid to move? or Shunt is a Necessary item which needs to be performed Before Cyber Knife?

2) Does the shunt normally removed after the fluid is moved to another part of Body or Shunt is inserted permanently in Body and we have to keep on changing it ?

I have read your query and here is my advice.

If the hydrocephalus is considered serious and there is an imminent life-threatening situation, the hydrocephalus should be treated first.

If the Doctor thinks that a temporary shunting technique may be used ( external ventricular derivation ) before the cyberknife, it makes sense, because it improves the symptoms of the patient and later the derivation can be removed.

In other scenarios, the tumor should be removed first, by cyberknife or open surgery.

This because if the ventricular-peritoneal shunt is placed, it should be maintained all the life.

On the other hand, shunt placement has some (rare) possible complications such as malfunctioning, infection, etc.

So what I understood from your response is 1)If we do Shunt before Cyber knife Shunt is temporary but if Cyber KNIFE OR surgery is done first then we will have shut for life ? is this the right understanding.

The confusion we have is shunt a temporary or permanent?

and After Cyber knife normally how much time normally it take to get the tumor cured 100% ?

The explanation is as follows:

If the Doctor planned to put first a temporary shunt (external) in order to improve the symptoms and to do the cyberknife, and after that to remove the shunt, it makes sense because the shrinking of the tumor after the cyberknife treatment may last for months, the results in the hydrocephalus symptoms could take some time to be seen.

It is my opinion that the tumor should be treated before the decision for a permanent shunting (if there's no evidence for serious complications from hydrocephalus), with the hope that successful tumor treatment will result in hydrocephalus treatment too (without shunting).

Again, a temporary external shunting before cyberknife makes sense, a permanent shunting before cyberknife should be reevaluated.

Doctor Sorry for one more follow up question which came up after discussing this with family is on type of shunt.

Our Doc said it will be a Internal shunt -Fluid will drain internally ,Does this kind of shunt Temporary?

If its Internal do we have to go to a shunt surgery again to take it out after a Month?

The shunt which drains inside is normally kept for how long?

In general, it is not removed once it is placed, and the patient stays with it all the life.
