                                            Question : Hello,
I have acne and I was put on Tazorac and Aczone with a high dose in spironolactone. I broke out extremely bad for that my face got soo swollen. My derm told me I was going to get worse before I got better but It got to the point where my face just got infected. So I had to go to my family doctor and he prescribed me 100mg of Doxycycline and the swelling and infection improved. It's been two months that I have been using Taz, Aczone, XXXXXXX and Dozy and I'm not getting anymore breakouts at the lower bottom of my cheeks. I think it's improving; however I'm still breaking out from my upper cheeks and I still feel some bumps underneath. I talked to my derm today and I thought the creams were doing their job, but she told me she recommends I use Accutane. I imagine accutane is just a much more powerful drug that will break me out until I no longer have anything left to break out from. I'm guessing it works the same way as these creams that I am using, yet, she recommends I still use Accutane. In all honesty, I'm just looking for a second opinion. Is this really necessary? Do I truly need to use Accutane ? If I keep using Taz and Aczone won't it eventually clear my skin if I use it for more time ? Maybe 6 months? Or Do I need to get on Accutane? I would like to avoid Accutane but If it means me getting better then I will risk it. However, since I broke out at first with these medications and until 2 months later I'm seeing some results I figured these were gonna do it as long as I give the medications more time. What do you think ?
I'm sorry for the long explanation. Thanks so much, Jysel

Detailed Answer:
Hello
Thanks for the query and detailed explanation.
A good explanation is always helpful.
Accutane is a retinoid and as correctly mentioned by you it can cause break outs however at the same time it's an excellent drug for long term response.
I would like to see a photograph of the involved area.
This will help me in giving a closer diagnosis and better way of management and whether to start accutane or not.
Kindly upload a photograph.
Waiting for your response.
Thank you.
