Posted on Tue, 27 Oct 2015
Question: My father is 82 years old he is on a number of medications and I am concerned about the interaction since he was just released from the hospital because of severe dehydration. atenolol 25mg. furosemide 20mg. lipitor 80mg. celexa 20mg. glipizide 20mg. lisinopril 20mg. metformin 2000mg. nortriptyline 50mg. trazadone 25mg. isosorbide monitrate 60mg. He was taken off the furosemide and isosorbide when he was in the hospital. The information I received from the hospital doctors was vague, when I questioned the doctors or nurses I was passed off to the next person. The follow up with his regular doctor didn't go much better. The night he was rushed to the hospital his blood pressure was 90/52.

These test were ordered by his doctor yesterday all were normal except his glucose was 212. sodium, serum, potassium serum, bun serum, calcium serum, vitamin d 25 hydroxy, vitamin b12, folic acid, serum, tsh, cbc + diff, glucose, random, ldl direct, hemoglobin a1c, alt serum. The hospital labs are unavailable to us as of today, it maybe 10/8 when they are available.

Brief Answer:
They take him off of medication as these may cause potential Hypotension!

Detailed Answer:
Dr. Hanif warmly Welcomes you to my virtual clinic HCM!
I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

Furosemide is the Water Pill-it makes a lot of urine and keeps the blood pressure low. Your father had a low (90/52) blood pressure when he landed into the hospital so if he was given more furosemide, it must have caused a more fall in blood pressure and we don't need this to happen on an already BP.

Isosorbide is a drug that is Vasodilator (meaning that it causes the blood vessels to expand). This leads to pooling of the blood and a significant decrease in blood pressure. Due to this reason they put him off of it too.

His 212 Blood sugar is high, but it also gets high in many people in hospital due to the recent trauma. His blood sugar had remained well in control in the past 4 months that is obvious from his Glycated hemoglobin levels (HbA1c you said came back normal). So I am not much worried for his blood sugar levels. All rest of the test you said are normal.

Have a wonderful time ahead. Hope I answered all of your questions. Let me know if you have any further clinical concerns or doubts. I will be more than happy to get back to you in no time. If satisfied here, please give a generous five star rating to help us assist our patients in a better way; and, please don't forget to 'Close This Discussion'.
For any disease you have, a timely follow-up after sometime regarding the treatment efficacy and to see if there is any need to change the treatment options is very necessary. So keep a follow up to see how you doing (Write down my name in the start of your question always & I will be with you sooner).
