Posted on Wed, 21 Jan 2015
Question: Respected Doctor,

I had exposure on 19th July, The lady was nude and I was wearing my underwear and the lady hugged me for some 5 minutes,That time I ejaculated, Not sure whether my penis touched her vagina or not. I sucked her breasts.

I have taken western blot test on 10th November 2014. It came negative. I have taken CMIA test again on 15th December 2014 which came Non Reactive.

However I am getting Throat Infection and got fever for which I was been prescribed Levoflax 500mg and pulmocef tablets.
My weight also reduced to almost 4 kgs in these 3 months.
Since last 2 days I got a small swelling on the lower portion of the head near neck region.

Kindly advice whether am free from HIV or Do I need to do further test?

Brief Answer:
You are free from HIV 1 and 2 beyond any doubt.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
You are absolutely free from HIV 1 and 2. Your symptoms of fever and throat infection are not at all related to HIV. A change of antibiotic will help you.

Western blot after 4 months and CMIA after 5 months were very much conclusive and nothing to worry about HIV.
Ejaculating while the inner wear was on, and sucking breast had very negligible risk for HIV.
So you need not worry.
Your throat infection could easily be treated with some other antibiotic. Get well soon.

1)Presently I don't have Fever and Sore throat. It went off after taking medicines.

But I have Dry cough,Dry skin which is kind of peeling in the Hands and also I went to Dermatologist for the small Lumps in my head and the doctor diagnosed it as furuncle in some hair follicles in my head.
I have attached the pictures of the Dry skin condition kindly for your view and comments.
Are these related to HIV.

2)I have not taken immunosuppressant Drugs or PEP , Still Do I need to test after 6 months for conclusive HIV result?

Brief Answer:
You are free from HIV.Need no further testing.

Detailed Answer:
Hi,
Welcome back.

Whatever may be your problem, you are free from HIV. Your tests are very much conclusive including the RNA PCR test. You need no further test for HIV.

Dry cough and dry skin are common problem during winter for anybody. Nothing to worry. They are not related to HIV. All your tests are within normal except a rise in neutrophil which could be due to the recent fever or throat infection. You can consult a doctor for dry cough. Application of coconut oil twice a day would be enough and your dry skin would improve.

Respected Doctor,

Thanks for your reply.

In one of the lab in which I took the HIV test its mentioned in declaration form and the counsellor told that the person has to test once at 3 months and one more time after 6 months which has caused confusion in me..
It would be better if the HIV testing protocol window period is standardized.
I have taken foracart inhaler for weezing problem and cough before exposure , Will that have any impact on my test results?
In my younger childhood days I used to have regular sinus problem which is mentioned as immune deficiency problem in some websites.
And people with immune deficiency has to test at 6 months is mentioned in the internet.
Kindly could you provide your inputs on the points above and Please tell whether my test result is conclusive?

Brief Answer:
Long term steroid inhaler may affect window period.

Detailed Answer:
Hi,
Welcome to HCM.
Don't get confused with various statements regarding window period of HIV. Those are mentioned to save guard themselves in case any litigation arises. Normal situation window period would not go beyond 3 months.

How long and how frequent you are taking Foracort inhaler. If it is for a short while for a week or so nothing to worry. It would not have any influence on your test. If you are on Foracort for a longer period there is a possibility that it can delay the antibody formation and thereby the window period of the HIV test. Treatment for childhood sinusitis had no effect on your test.

Brief Answer:
Steroid inhaler used in 2013 not affect the window period during 2014

Detailed Answer:
Hi,
Welcome BACK.
No. Fortacort inhaler taken during 2013 would not have any effect on HIV test done after July 2014.
You can very well rely on the test and the test would be conclusive by 12 weeks after the risky sexual act.

3)I was not able to sleep and I have requested the lady with whom I had exposure and did a complete Health check up for her
which included HIV test also and the CMIA came Non reactive for the lady(After 143 days of my exposure). Her report is with the name
"December-Lady Report" kindly for your view. But I don't know about her past expsoure or about her steroid inclusive drugs etc.

4)I am worried about the symptoms am facing.
But the symptoms am facing is worrying me as I never got such kind of symptoms before the exposure.
If I am negative then why such kind of symptoms are showing up.

Brief Answer:
Rare possibilities and deviations can not be taken into account.

Detailed Answer:
Hi,
Welcome back.

In that case itself they had mentioned that it was very rare instance.
Every Human body can react differently to the foreign proteins. Severe in some, mild in many and nil in rare cases. Some congenital deficiency in that individual's immune system might be present.
But in my experience of 25 years with HIV and patients with HIV I had not come across any such instance. I had attended many national level and international level conferences and not heard such case from my colleagues and experts. It is not common with an normal individual like you.
