Can HIV Be Contracted Through Non-penetrative Sex?

Question : Hand jobs and HIV risks:
1) I have had 7 handjobs during massage at SPA during past 1.5 years. The recent ones being one on sep 7 and other on sept 27
2) I have not been exposed to therapists vaginal fluids or saliva in any of these encounters.
3) On sep 7 incident I shaved my pubic hair around my penis and balls just before the visit to SPA (probably 1 hr before). I asked the masseuse for a hard pressure massage as I was having body aches. After massaging my body for half an hour she gave me a hand job with warm massage oil and I ejaculated on my body. I did not notice any blood during the incident. After 3 weeks of the incident I started having many symptoms like fatigue, body aches (neck and back), chills, small rashes (tiny red dots in patches) which disappear in 1 day and re appear elsewhere. No fever (only mild fever initially for 1-2 days) but my body gets hot during the sleep and I wake up all of sudden. Lot of anxiety and nightmares. I lost my appetite in this week which I eventually regained by the end of the week. Second week the symptoms were mild, however I my anxiety and depression grew. By the end of this week I noticed a lymph node in my groin area. It is not painful or tender. I also have constant itching in my groin may be due to fungal infection. Could this have cause the swollen lymphnode?? I have noticed any lymph node under my armpits and neck. I repeatedly keep checking my neck for lymph nodes and now my neck appears to be swollen. I am confused whether it is a lymph node or muscle. It has been over a month now and now I have mild sore throat and mild fever and stuffy nose. I have started smoking a lot everyday after quitting for nearly 2years. I am unable to sleep at nights and afraid to test. My concerns are:
1) I have never checked the hands of masseuse during the massages and if they had any tiny cuts or lesions on hands which may bleed due to friction in the massage and comes in contact with tiny cuts or abrasions on my body am I at risk?
2) Does my shaving of pubic hair just before incident increases any risk. I did not notice any major or bleeding cut but may got small nicks or scratches from razor.
3) Any small abrasions or pimples present on my body would increase threat?
4) Does the presence of massage oil will decrease or increase the risk? Will it completely kill the virus?
5) Also please answer my question about lymph nodes I mentioned earlier.
I am 100% sure that I never came in contact with vaginal/oral fluids. All my worry is that I never bothered to check the hand of masseuses during all my encounters. I strongly feel that I would have noticed if there was any active bleeding going around. But I never noticed anything.

No fluid - no HIV

Thanks for writing to us. I can understand your concern.

As you are 100% sure that there is no contact of vaginal/oral fluids there is no chance of HIV transmission.

1. Though you did not check hands of masseuses or abrasions on your body no chance of transmission of HIV. Hiv transmits through body fluid of hiv infected person to exposed mucosa of the normal person.

2. No shaving does not increase the risk until there is gross fresh abrasion exposing the mucosa and fluid of hiv infected person comes in contact with that.

3. Pimple or abrasion would not increase the risk.

4. Oil may or may not kill the virus depending upon density of virus if at all it is there.

5. Your groin lymph nodes can be due to fungal infection or after shaving as sometime after shaving skin infection is common if hygiene is not properly taken care. You should be examined by a doctor for lymph not enlargement to clarify that is it lymph node or not.

If your anxiety is excess and affecting your life, you should consult psychiatrist once for counselling and /or short course of anti anxiety medicine.

Thanks for the response doctor.

I have few more concerns. Please help

1) I am circumsized long back probably 15 years ago. Does this have any impact?

2) Please comment on my symptoms mentioned and how closely they mimic infection.

3) You mentioned cut exposing mucosa. May you please explain this a bit. Does small shaving cuts on testicles give exposure to mucosa?

4) Do the lymph nodes hurt or they can be non tender as well? I have also noticed a lymph node near my right elbow.

circumcision has no impact

Hi,

Thanks for follow up.

1. No. Circumcision does not have any impact on transmission of HIV.

2. Symptoms like fatigue, body aches (neck and back), chills, small rashes (tiny red dots in patches) - These all can be due to flu like viral infection. It disappeared in one day so not at all related to HIV.

3. Mucosa is soft surface tissue. Like skin n oral cavity is called bucal mucosa and skin of glans penis beneath foreskin. When normal body skin abraded it exposed light pink soft layer of skin which is called mucosa. Mucosa can absorb fluid and that is why HIV. No small shaving cuts do not give exposure to mucosa.

4. Lymph node can be non tender also. Lymph node ear elbow is less likely, it is likely muscle tissue.
