Can HIV Be Contracted Through Non-penetrative Sexual Activity?

Posted on Wed, 17 Aug 2016
Question: I played in prostitute`s boobs by hand then i slept on her and we rubbed against each other. I was wearing my underwear and trouser as well but they were too thin as it is summer now, the girl was wearing her underwear as well. I rubbed my penis on her vaginal area without taking off my underwear and she did not take it off as well.I ejaculated my semen while we were rubbing against each other and later my underwear was wet and i can see that, in case she had vaginal fluid at the same place where i ejaculated ( both of us are wearing underwear)and she was HIV+ , could the HIV penetrate her underwear and my underwear and transmit to me ? what about other STD diseases? should i make HIV test?

Hello Doctor..... Can you mention to me the best time to apply PCR test after sexual intercourse? I know that the virus can be detected after 11 days of exposure. giving more days would be better to get 100% result without mistakes? for example 30 days and above. The same story to Hepatitis, i heard i need to be wait for 21 days before applying PCR test .... hope you give me the best time to apply PCR test for STDs . The more i wait the more accurate results i get in case of PCR test?

Brief Answer:
Your HIV risk is negligible only.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
HIV transmission is facilitated by a breach in the skin or mucus membrane and when there is an exchange of body fluids through this breach or some other way. It can not be transmitted with just a contact with an intact skin. I am handling with HIV patients since 25 years or more and even worked with children affected with HIV in care homes. During this tenure there were many occasions where I had come across their secretions or came in contact with healing sores. Similarly in a family who cares HIV individuals such incident are common. But we care providers remain HIV free so far. HIV can not spread through intact skin or with rubbing of genitals with inner wears intact even if it becomes wet during the act. Your risk for HIV is negligible.
Regarding the PCR tests, even though it can detect the presence of an infection as early as 7 days, it will be considered more conclusive by 4 weeks after the high risk contact.

I cannot see any bleeding in my skin but maybe my skin is scratched and some red areas but there is no bleeding,in this scenario am i at risk ? And this scratch can be considered as a bleeding skin? Can u explain to me please what do u mean in breach in the skin and bleeding area exactly? Let me give an example please, if i get insured in my hand 1 day or 2 days ago then the bleeding has stoped but u still can see the injured area without bleeding then if it came in contact with fluids or blood which are containing HIV virus then the virus will transmit to my bloodstream during this scenario? Thanks

Brief Answer:
Wound till complete healing has negligible risk when contact with fluid

Detailed Answer:
Hi,
Welcome back.
Any wound (abrasion or cut) till complete healing (till become normal skin) immaterial whether it is bleeding or not, it is considered that injured area has risk for HIV or any other blood born diseases, when it came in contact with the infectious material. But this risk is negligible only when compared to vaginal or anal intercourse as per CDC XXXXXXX United States of XXXXXXX

Thanks doctor,
I washed the underwear and the trouser twice by washing machine, I dont dare to wear them again as i feel they are infected and might be the virus on them, should i wear them again with confidence that nothing will transmit to me after washing them twice by washing machine? On the other hand yes the CDC website says that the window period for HIV is 7 to 28 days if i would test by using PCR , so the best period of time is 28 days after risk exposure, what about after 28 days? Let us see 30 days and above ; the result will not be reliable nor accurate? One of the doctors in my city said at 32 days i should not do PCR test as the result will not be reliable and conclusive, he said i must do it between 7 to 28 days ; his words are right or not doctor? Others say after 3 weeks the result of PCR will not be changed at all .. I am a bit confused really. ,

Brief Answer:
HIV PCR remain positive throughout the course of the disease from 3-4 weeks

Detailed Answer:
Hi,
Welcome back.
HIV and STD causing organism are not viable outside the body or body fluids not more than few seconds. So your washed and dried inner-wears are no longer infectious and there is no harm in using the same.
HIV PCR can detect the presence of virus in case of an infection as early as 1 week and can be considered conclusive by 4 weeks. After that qualitative PCR will be positive for ever and the quantitative PCR will show undetectable following successful treatment and while on treatment (life long treatment). PCR results are highly reliable up to the extent of 99%.
I hope that your questions had been answered.
