Can HIV Be Contracted Through Oral Sex?

Question : Hi Doctor

I was recently (5 days back) on a business trip in hong kong and went for a massage session. Post the massage, the lady gave me a hand job and a blow job (with condom). During this, I also kissed her vagina and performed oral sex for a short time (< 20 seconds).

What are the chances of HIV infection? I dont have any symptoms as such but I would want to rule this out and hence could you suggest any suitable tests for an early and confirmatory resolution to this issue

Massage activities and hand job carry almost 'nil' risk as far as HIV is concerned as there is no exchange of body fluids happened.
Oral sex carries a low risk. Infected blood transfusion, needle sharing, anal sex and vaginal sex are considered to be high risk for HIV transmission whereas oral sex carries a low risk as per CDC, XXXXXXX USA. The risk through vaginal sex is around 4-8 per 10000 exposures. It is very much lesser with oral sex. Then you can imagine your risk.
HIV can be ruled with antibody detecting tests 3 months after the risk activities conclusively. PCR test for HIV 1 and HIV 2 (available separately) can rule out this possibility more conclusive by 3-4 weeks, even though it can detect the presence of infection as early as one week after the exposure.

Hi Doctor

Thanks for your response. So do you recommend that I do a PCR Test at the end of say 12 days as well as at the end of 4 weeks to rule this out?

Would these two be sufficient or is there a possibility that both of the above tests come out negative and I still get a positive detection through an antibody tests at the end of 3 months.

I also had a question on "The risk through vaginal sex is around 4-8 per 10000 exposures" - is this in cases where the other partner is HIV positive or a blended ratio of when one does not know the status of the partner.

Also, as per your knowledge, with respect to HIV via oral sex - is it an academic possibility or have there been documented reported cases of oral sex leading to HIV?

Welcome back.
We had seen one or two cases of HIV acquired through oral sex during my carrier of More than 25 years in HIV practice. The statistics given by CDC is referring to sex with known HIV population.
Negative Separate Qualitative RNA PCR tests for HIV 1 and 2, 4 weeks after the exposure is conclusive. No need to go for further antibody test after 3 months.
