Can HIV Be Contracted Through Protected Sex During Menstruation?
Question: Hi! 15 days back I had below exposure.
I had gone to a massage parlor where I had sex with the massage girl. I had used 2 condoms. She gave me blow job with the condom. We had intercourse. After finishing the intercourse I took out the condom to check if it is broken or not. The condom was intact. But then I noticed few blood drops on my index finger. It was her menstrual blood.first she told she was on her periods. Then she said her period got over 4 days back. She didn't give proper reply. I am really really worried. I don't see any cuts on my finger. Is there any chance to contract HIV? I am not sure if blood was anywhere else on mY hand. I touched the blood approx after 20-30 seconds I feel. Pls help.

thanks for your reply. I was so panicked at that time. I dont think i washed my hand with soap. i washed just with water. After reaching home after 20-25 mins, i washed my hands with soap.
To add...i was nude when i noticed blood. i dont remember what i did after that. i was very tensed. i talked to her about blood for 15-20 seconds. weared my clothes n went out to wash my hands with water. then after driving bike i went home and washed hand with soap after 20 minutes. Thanks doctor for your reply. This answer is assuring to me. However there are two more incidences which happened in the past 45 days. Details are below:
1) The above described event happened on 16th nov where I touched the blood.
2) 1 month prior to this I.e.on 16th Oct I had sex with prostitute. I had 1.5l of beer. Not very drunk.but during sex condom slipped off 2-3 times. I was sure I had put condom before inserting. However after sex the doubt's came if I had unprotected sex. Though after some time I convinced myself that everytime condom slipped off I had put it back and then inserted.
3) on 19th Nov I met a girl online and went for a movie with her. There we had smooch 2-3 times n tongue kiss 2-3 times for very short duration. Say 5 seconds XXXXXXX for each kiss. However when I was kissing her ears due to her earrings I got a small cut in my tongue. Not sure if it was bleeding. Might have done tongue kiss once after that.
My problem. On 20th Nov I saw some pink rashes on my chest. Especially when I scratch they became prominent. On fist small pink rashes were there. They lasted for a week and now they r almost not there. Also since 20th Nov I have lost 2kgs weight. I am not sure if these are early HIV symptoms. These have scared me a lot. I will get tested after 3 months but living even 1 day is difficult for me. I tested for pcr RNA and HIV elfa on 20th Nov. Both tests were negative. Do these tests give any assurance for at least the incidence which happened on 16th Oct. I was not worried about 16th Oct incidence but after symptoms I feel I might have done unprotected. Can these symptoms be for 16th Nov incidence.

Let you know about HIV infection: In an unprotected intercourse with a HIV positive person the HIV viruses gets entered into the body and divides in innumerous numbers (millions of viruses per ml).
After a period of 2-6 week there is mounting of antibodies against the viruses. These antibodies can take at the maximum of 8-10 weeks of the period to get expressed. Therefore the infection can be analyzed by two ways -
-A): By HIV virus detection: done by RNA- PCR. It tells us the number of copies of virus present in per ml of blood. By the latest of methods the viruses can be detected within 10 days to 2 weeks of period.
B): By HIV antibody Detection: AS told earlier once the antibodies appear in 8-10 weeks then these antibodies to HIV virus persist throughout life.
Now correlating all the facts and interpreting with your situation:
1. The very first thing of most help is using a condom. It is rarest or rare chance in our medical science to get HIV infection despite using condoms that too by improper use. This is one of the safest methods which we know in this world.

1) i understand using condom is safe. however in 16th oct incidence as i was drunk i might have inserted without condom. I used condom but as i said it slipped 2-3 times during intercourse. everytime i put it again during intercourse but as i was drunk it might have got slipped off during intercourse or i might have put without condom. Though i was not drunk a lot but still this thing came to my mind. This is not safe right? What are the chances of hiv here?
2) during 19th Nov kiss incidence, the girl was of unknown hiv status. She looks very odd and different. she has many scars on her face and from her lifestyle i am sure she involves in very risky behaviour. if she had herpes or sores in her mouth can blood exchange and i can get hiv? what are the chances of other stds?
3) for 16th Nov incidence where i touched the blood, what if some blood got into minor cuts which i am unaware of or about small cuts which are near nails when little skin comes out? u asked if i washed my hand and after how long. what is the significance of that as i didnt wash immediately and i weared clothes after touchung blood so it might have got wiped out due to that?
regarding symptoms i have following:
1) rashes which reduced after a week
2) dry lips n feel little buring in mouth when i eat sometimes though dont see mouth ulcers.
3) little chest pain which may be muscle pain.
4) very mild white spots near eyes and forehead.
5) weight loss. this can be due to anxiety but i was anxious and very tensed for an year after break up though i didnt loose weight i rather gained. i feel hungry quickly but wheneevr i eat i dont feel i am getting energy from it or it is going in my body. why all these? these are not any ARS symptoms?
6) i feel mild fever and hot air in my throat though not everytime during the day.

For the part of 19th Nov kiss incidence: If kissing was done after the cut then also it is not a risky behavior in terms of HIV. HIV cannot get spread by mouth kissing.
Even then if you want to go for testing then RNA PCR can be done again after 5th December. You need not to wait for 3 months. A RNA PCR test is sufficient to declare your healthy status and would be coming negative.
There can be meager chances of Herpes and hepatitis. For Herpes you can go for testing at 28 days and for Hepatitis you can go for testing at 60 days period.
After going through all your symptoms: you can see that there is no fever in enumerated symptoms. It is the fever of high grade which is most important sign of acute retroviral syndrome. Rest of all the symptoms are basically anxiety based, so reduce your anxiety.

Hello Doctor! Thanks for your responses. It is really convincing. I have done few tests on 3rd dec which are below :
1) Chlaymydia Antigen- Negative
2) Hepatitis C Antobody- Negative
3) Urethra swab culture for gonnoreha- negative
4) HIV1 DNA by PCR- Negative
5) HIV1 and 2 ELISA- Negative
HbasAg,VDRL, HSV1 and 2 Igg and Igm were done on 20th Nov. all were negative.
My question is can i consider myself totally free from above STDs or do i need to repeat these tests as i have used CSWs prior to these 3 incidents also. If yes, which tests i need to repeat and after how long?
my other problem is that right from 21st nov i am experiencing dry lips. I know this is winter season but i dont know why suddenly they are getting so dry. Since past many years i didnt face this thing. 3-4 days back my upper lip got cracked. Though blood was not coming out but i could see blood. There was slight pain when i was trying to open my mouth and next day one pimple kind of thing appeared(on 6th dec) on my lower lip near the mouth corner right side which was paining a bit. Little itching is there below lower lip till now n it is little reddish. This pimple or blister went next day n very mild pain was there. Now almost no pain. sometime i feel little pain is there in left jaw or right jaw but it goes within minutes. I am very scared about this dry lips and that pimple. are these symptoms of some STD? I read several online materials and i am scared about oral herpes. Do i have oral herpes? How can we find that? can oral herpes blister go away in one day?
If you suspect oral herpes, after how long i can repeat HSV1 and 2 to get sure result?
Kindly help with these.

HbasAg,VDRL, HSV1 and 2 Igg and Igm were done on 20th Nov. all were negative.
My question is can i consider myself totally free from above STDs or do i need to repeat these tests as i have used CSWs prior to these 3 incidents also. If yes, which tests i need to repeat and after how long?
my other problem is that right from 21st nov i am experiencing dry lips. I know this is winter season but i dont know why suddenly they are getting so dry. Since past many years i didnt face this thing. 3-4 days back my upper lip got cracked. Though blood was not coming out but i could see blood. There was slight pain when i was trying to open my mouth and next day one pimple kind of thing appeared(on 6th dec) on my lower lip near the mouth corner right side which was paining a bit. Little itching is there below lower lip till now n it is little reddish. This pimple or blister went next day n very mild pain was there. Now almost no pain. sometime i feel little pain is there in left jaw or right jaw but it goes within minutes. I am very scared about this dry lips and that pimple. are these symptoms of some STD? I read several online materials and i am scared about oral herpes. Do i have oral herpes? How can we find that? can oral herpes blister go away in one day?
If you suspect oral herpes, after how long i can repeat HSV1 and 2 to get sure result?

For the queries above:
1) Chlamydia Antigen- Negative: It is confirmatory to declare it negative as most of the times it is detectable within 7 days of infection.
2) Hepatitis C Antibody- Negative: you have to go for the test at 60 days of the period for both hepatitis B and C as the antibody test done within 15 days is not confirmatory. The antibodies for both hepattis B and C requires minimum of 60 days or longer to be detectable.
3) Urethra swab culture for gonorrhea- negative: It is confirmatory. A test done after 7 days is quite reasonable and we can say that you are free from gonorrhea.

An RNA PCR is more sensitive than DNA PCR in your kind of cases. SO it is better to repeat it with RNA PCR any time after 2 weeks or can be done by DNA PCR after 21 days of exposure.
5) HIV1 and 2 ELISA- Negative: It is of no significance as antibodies take minimum of 6-8 weeks to be detectable and are best confirmed at 3 months. So for the same you have to wait till 20 February if want to go for ELISA. But in case of RNA PCR is negative after 21 days then there is no need of this test.

Thanks doctor. I have attached a picture of the lip area for your reference. Kindly have a look at that. Today below the lower lip in the middle I found very small yellow fluid blister. Very very small in size but when I touched it it was there. After few minutes that was gone. Not sure if I broke that. I fear that in the next few days that may become a blister. As I explained you the basic signs and symptoms of herpes, you can find out that the area has neither multiple blisters nor you have give any history of pain or soreness around any of the areas.
In general herpes occurs by 7-14 days of period. In your case it has almost 21 days since the incidence so it is unlikely to get herpes.
There are two antibodies for the detection of herpes: one is IgM and the other is IgG. IgM starts appearing by 10 th day and disappears by 24 to 28 days. On the other side IgG may appear by 28 days to 42 days of the period. So clinically when we test for these antibodies then analyze the condition accordingly.
Anyhow as I said I am pretty sure that at this time there are no signs of herpes. And getting through the time frame as well as the exposure I am quite sure that you would not have the same. So there are least of chances for herpes in your case.
For Hepatitis: DO let me know if you are vaccinated for hepatitis or not. Again in your case there are negligible chances of the same as per your type of exposure.
For RNA PCR: It is the two weeks of time form the day of exposure. The results of DNA PCR are very much encouraging and I am almost sure that you are free form HIV too as per my clinical experience.

sorry for long post but kindly help me as you have been doing as an elder brother to me as i am really nervous. your every guidance matters a lot to me due to your excellent patience, knowledge and guidance.
I am really really nervous for herpes. I don't know what to do and how to conclude on this. i know you have reassured that so far it doesnt look like herpes but below things are making me too much worried.
1) the deep kiss incidence seems the culprit to me right now. to recall the deep kiss happened 2-3 times for 4-5 seconds each. french kiss happened 2 times for 3-4 seconds each. so total may be 20-25 seconds in all with few minutes gap. The girl whom i kissed is the most weird looking girl i have ever seen in my life as i told before. she is from XXXXXXX her face skin is very weird and looks as if her entire face is burnt or plastic surgery is done. i am not able to guess her age also as she looks very odd. may be middle age or what. from her discussions m sure she engages in very risky behaviours. so m pretty sure she has herpes.
2) considering her positive status i have read that herpes is very contagious so i am very scared that chances of getting infected are high

Can few seconds kissing transfer it?i can see your confidence level is high that i wont have hiv but may be not that much for herpes. am i misunderstanding you or due to high contagious nature of virus you are not sure?
2) I wanted to understand statistics for transfer in the kind of kiss i described above. in the last post you mentioned 1 in 10000 for a trivial touch but i feel by trivial touch you misunderstood my exposure to blood touching incident whihc happened on 16th nov. kindly help
3) my igm was 0.583 and cut off was 0.8. very close whereas IGG was still well below cut off may be becuase 4 weeks is early. this close value to cut off indicates something?
4) is there any test like hiv rna pcr which i can do for hsv so that it will be quick and can boost my confidence a bit?
5) what is the best time to do blood test for hsv1 and 2? is really 12-16 weeks required to get confirmation?
i asked the girl 2 days back, if she had cold sores. her reply was no. i asked her did you get tested earlier for hsv when you got tested for hiv. she said she didnt get test and became rude. she asked me to get tested and not to talk again. i dont know how she knows about herpes if she is so clean and how come she never got tested for hsv if she engages in risky behaviours and got tested for HIV.. i think she is lying.

@ We so often meet people and at-least shake hand or hug them. Hoe many of the times you have seen whether the other person has herpes or not. So do not just rely on statistics but also know the practical aspect fo life where we so often have skin to skin contact. We don't get herpes or any related infection by these kind of skin contact. So again relax for the same.

@ The value of IgM or IgG if below cut off limit then is of no significance. No matter how close the value is, it should be below cut off value. So a value of 0.58 is way much below 0.8 and therefore of no significance.

@ Yes there is Real time DNA PCR for HSV1 and 2. You can ask a reputed lab near you to do the test for you.

@ The value of IgM or IgG if below cut off limit then is of no significance. No matter how close the value is, it should be below cut off value. So a value of 0.58 is way much below 0.8 and therefore of no significance.
4) is there any test like hiv rna pcr which i can do for hsv so that it will be quick and can boost my confidence a bit?
@ Yes there is Real time DNA PCR for HSV1 and 2. You can ask a reputed lab near you to do the test for you.
5) what is the best time to do blood test for hsv1 and 2? is really 12-16 weeks required to get confirmation?

2) since past 3-4 days i am again experiencing little buring near my upper and lower lips. lips are still dry though better than 3 weeks back. mouth corners are little cracked. inside my upper lip i feel small blisters near corner of mouth on both right and left side of upper lip. though they are not big and dont pain.nothing. i cant see them also. but when i use my tongue to lift the upper lip then i see them. when licking i feel little unusual but not bumps. i hope these are not related to herpes and nothing to worry about?

Thanks sir! Just some time back recieved RNA PCR report. I gave blood on 18th Dec which is around 1 month from my stupid and senseless kissing incidence. Fortunately, it has come negative.
First question. Your assurance that in your experience you have never seen any positive herpes cases in my kind of exposure has given some relief. But i wanted to know there were cases in which other partner was hsv and hepatitis infected and still the person didnt catch hsv or hepatitis? Kindly answer this for my assurance.
Also, there are few things which have scared me a lot. i have stopped reading on internet but i read few things before.
1) there was an XXXXXXX XXXXXXX guy who had unprotected oral and deep kissing with just 3 prostitutes. his symptom was one pimple or few blisters i dont remember. probably he had few blisters. no itching or buring. that went away in one day. only red mark remained which became prominent only while bathing. it went after a week. he tested positive after 6 weeks. this scared me to hell. people say fever should come and blisters are painfull still in his case nothing was there and he turned positive.
doesnot this case make me also susceptible that i may have something? symptoms can be mild right?
2) my upper lip right corner got broke and lower lip had that pimple. can herpes happen like this where one lip is broken and other has pimple? how suddenly skin got broken during pimple? can herpes happen like that?
3) i had read there is a stage called crusting in herpes. the picture looked like mine when my pimple went away next day. that stage comes in normal pimple also?
4)I will have another test for IGG. 6 weeks is good enough to be 100% confident as you said 42 days or 8 weeks is better in my case?

Before I answer any of the queries let me first guide you that herpes is one of the simplest viral infection which is never life threatening. It is due to undue over-apprehension by the western world that one gets terrified so much. Therefore my dear friend let you just relax about any of the issues regarding herpes.

since yesterday i am feeling feverish and tired. today i have fever and headache.I am feeling tired and weak. my legs especially thighs are paining. i was having breathlessness since past few weeks but it has increased a bit in past few days.
i wanted to know if i can take medicines like antibiotics or Ibrufen, combiflam etc for this?

You can take a pain killer and it would not be hampering in the test result of any antibody test. None of the pain killer medicines affect the test result of any antibody tests.
