Can HIV Be Contracted Through Protected Sex?

Question: Dear All, 
I am 38 years old male had got following sex history: 
On 1st January 2014 I had unprotected sex at Istanbul CSW. 
On 12th January 2014 I had another sexual encounter with a different CSW. This time condom slipped and after finishing sex only we noticed. 
On 23rd May 2014 I had next sexual encounter with another CSW, this time it was protected sex but with open mouth deep throat kissing. 
I have done HIV Antibody test on 30th September 2014 with negative result. 
On 27th December 2014 I had another HIV test using CMIA method. Reading was 0.17 and negative. 
To get confirmed I did another HIV test on 4th January 2015 using Tri-Dot method and result was negative. 

Then I was very much sure about my status and had no further sex except with my wife. After having sex within 4 days my wife had flu with high fever. After 2 days I also had same kind of flu. Now I have persistent pharyngitis with dry cough and Diarrhea
I am in utmost tension about possibility of Late seroconversion. There is no reason for this kind of diseases. 
Is there any possibility of Late Seroconversion? 
When Can HIV antibody test be 100% confirmatory? 
My last test was more than 7 months post exposure. 
Can Acute Retro viral Symptoms appear after 5 days of exposure? 
Can persistent granular pharyngitis a symptom of HIV?

I have gone through your query in detail and i have understood your concern.

Hiv has been adequtely ruled out.

Current guidelines state that a negative Hiv antibody test at 3 months is conclusive and thereafter there is no need to do any further tests in otherwise healthy individuals.

In very rare cases there may be late seroconversion and such scenarios include patients with immunodeficiency e.g organ transplant recipients, patients on Oral steroids and those who were offered PEP i.e post exposure prophylaxis.

Anyways, ARS i.e Acute retroviral syndrome usually appears a week after Hiv infection.

Therefore in my view the flu like symptoms in your wife and later in you were entirely due to an unrelated cause.

I did these tests from local pathology lab of XXXXXXX One of the lab was serum analysis centre pvt. Ltd. Are these reliable?

Some doctors in health care magic saying antibody test is not much reliable and go for direct antigen test. Is it true?

Antibody based tests are reliable in your case

Hi.

Antigen testing is only advised if there is a short time interval between sexual contact and laboratory testing because antigens can be detected early by lab testing whereas antibodies take time to appear in circulation and therefore may not be detectable soon after Hiv infection i.e antibodies have a window period of detection

This window period for antibody based test is 3 months in case of HIV and therefore after 3 months there will be circulating antibodies in all Hiv infected patients.

Since in your case the last antibody test done was more than 6 months after your last risky sexual contact therefore the antibody test is reliable in your case and there is no need for antigen test.

In XXXXXXX are the private pathology labs are reliable? I did one of the test from Serum analysis pvt Ltd. They r iso 9001 certified.

Yes they are reliable. The reliability of a test is dependent on the test kit/ method. TriDot and CMIA are latest 4th generation tests and are reliable.

My doubt, Is there cases where pathology labs generating report without conducting tests. Can it happen? As you are dealing these kind of cases, you can tell the exact story.

Also I am suffering from persistent Pharyngitis. I have deviated nasal septum. Also persistent post nasal drip also Sinus problem. I consulted with ENT specialist and they said it may be due to DNS. My right nostril is mildly congested
Can DNS lead to persistent Pharyngitis?
I am doing regular gargle, taking Lasma LC and Memoflo nasal spray but not working.
