my penis and hand with same water. Then after 20 min we again moved for 3 rd session. Again she washed her vagina and hand with same bucket water. This time water is very less in bucket. After that I washed my hand and penis with same water. During that night I saw a Paper in her file written as STD and some medicine was written there. I asked her about that but she hidden me and that is just an infection in my vaginal area. After 22 day of my exposure I got flu like feelings but when temperature measured it was 98.6 always. Fever stays upto 4 weeks l. In between that. I got sore throat which also stays 5 weeks. Then I got severe muscle pain and tingling sensation on my palm and sole. Also skin rash is there in my palm and sole. These skin rash are felt like hot sensation. I have lost my weight around 4 kg. I have white layer on my tongue and white lining on my side of the tongue also ulcer in mouth. All the symptoms are still existing (11.11.15). After 18 day of my exposure I did real rime pcr, quantitative (chenai -shankar netralaya ) result was not detectable. I did hiv cmia test on 30th,38th,53th and 61 st day of my exposure and all are negative. It might be due to window period. I am waiting for my 90 day test result. I visited a doctor here after getting my throat infection he suggested some medicine after taking that I got irritating feelings on my hand and feet sole. Then I stopped that medicine and again went to same doctor again he changed some medicine but to off no result. Now my throat infection is some how ok. After taking hot salt water gargling. Then I went for another doctor for my muscle pain and tingling sensation but do not tell them about my incident. He suggested for cbc, urine culture and liver function test and found every thing ok. Then he said some viral infection might be there and suggested some medicine.After 60 day of my exposure I have tested for dengue, hepatic b and c , very and prp but everything ok. Now I am with mind that I am hiv positive. I do not know the accuracy of CMIA hiv test results obtained from Dr lal path lab. In UK it is considered as conclusive after 28 days. But here in XXXXXXX all are saying result will be accurate after 90 day. Sir in my 52 day cmia hiv test the value is 0.07 and 61 day value is 0.46. Will it be above 1 after 90 day ? I have asked Dr lal path lab that why they are not mentioning p24 value , they said that , that value is for both. Is it like that ? Or in test report p24 value and antibody value written separately? Should I trust the result obtained from Dr lal path lab.

You had protected exposures. The risk of HIV by using the same water from a bucket for washing the genitals by you and your sexual partner is almost nil. HIV can not survive outside the body fluid not more than few seconds. It will not survive in bucket of water.
All your symptoms including throat infection, fever, body pain and mouth ulcers are not specific to HIV infection. They are quite common symptoms for anyone to get in the absence of an HIV infection. Close intimacy during sex facilitates the transmission an ordinary viral infection through air (breath, cough and sneeze).
Regarding your test reports, Lal path lab has a good reputation and their tests are highly reliable. Undetectable viral load with RNA quantitative test is useful to assess the prognosis of the infection rather than for diagnosis. RNA Qualitative test or DNA PCR test would be of more use than the RNA Quantitative test, that too, of more value by 4 weeks than with 18 days.
Regarding CMIA tests, if it is forth generation test (COMBO/DUO), the nonreactive test done after 28 days had ruled out the possibility of HIV 1 infection beyond any doubt as the P24 part of the test became conclusive by 4 weeks after the high risk exposure whereas the antibody part of the test would become conclusive by 12 weeks after the window period. HIV 1 infection is more common throughout the world except in certain pockets of XXXXXXX to the extent of more than 99% (either in UK or in India). HIV 2 is very very rare possibility and can be ruled out by any HIV test done after 12 weeks post exposure. Upload the copy of the test report (CMIA), to verify whether it is a 3rd or 4th generation test.

1. As you rightly pointed out in early infection the viral load must be very high. But that is during seroconversion period (3-4 weeks upto 12 weeks). Antigen can be detected by one week on wards. But in rare occasions it may be low due to late in establishing the infection inside the body. So the PCR test is more reliable after 3-4 weeks.
2. DNA PCR is very much conclusive by 6o days as far as HIV 1 infection is concerned.
3. Symptoms are unlikely to due to HIV. Consult a good physician.
4. Your CMIA test are 4th generation COMBO tests. These negative test totally ruled out the possibility of HIV 1 infection beyond any doubt and they are very much reliable.

DNA PCR 4 weeks after the high risk activity is reliable to the extent of 99.9% and it is conclusive that you are free from HIV 1 infection. Your symptoms are not due to HIV 1 infection. Rarest possibility of HIV 2 is yet to be ruled out. It is quite unlikely. Consult a good physician for your present problems and get well soon.
You have repeat the Rapid test (Antibody detection test) or 4th generation test once again to rule out HIV 2 infection, if you have any doubt about the relationship of these symptoms to HIV infection, 12 weeks after the exposure. It is quite unlikely.

Your DNA PCR is also not detecting any HIV particle. So it is extra confirmed that you are free from HIV 1 infection including all sub-types. Not to worry.
Western blot by 10 weeks will be able to detect antibodies as almost you crossed widow period. But it would be more conclusive by 12 weeks. As HIV 1 had been ruled out already Western blot is not necessary for you. Regarding HIV 2 infection, it is as equal to any other rapid test. So rapid test would be adequate. It would be conclusive by 12 weeks. Regarding your symptoms would be unlikely to be due to HIV.

2. HIV 2 is present in XXXXXXX The prevalence is less than 0.5%.
3. Syphilis can not be transmitted through contaminated water (washing in a common bucket). Syphilis can be detected by RPR (rapid plasma reagin) test and will become positive by 4 weeks after the exposure in the presence of infection.
You can go for any antibody test 12 weeks after the exposure. That also would be likely to be nonreactive.

Laboratories want to protect their skin from medico-legal problems. So their instructions sometimes are not necessarily taken as absolute. As far as my knowledge goes you need not bother about other serotypes of HIV 1. You are free from HIV 1 infection.
HIV 2 RNA PCR or DNA PCR by 4 weeks on wards can rue out or confirm HIV 2 infection.If it is available you can very well take up the same. But I feel that you need not worry about HIV infection with this exposure. CMIA by 12 weeks can rule out both HIV 1 and 2, can not be taken as 100% conclusive by 10 weeks.

There is no PCR test common for both HIV 1 and 2 infection. Qualitative RNA PCR tests are available for HIV 2 with some bigger labs like SRL Ranbaxy or Lal Path etc. DNA PCR is available with many labs. Instead of looking for HIV tests you can meet a good physician and get well soon. Chances of seroconversion for HIV for you by 90 days is remote.
