Posted on Fri, 22 Jan 2016
Question: I am 28 M and my partner was 30 F. I was engaged in unprotected Oral sex (Received Fellatio) from her. its been 10 days now. It last for only 1 min and during this I did not remember i have ejaculated in her mouth. Considering she had multiple partners, what are the chances to get affected with all STDs , HIV, HPv etc. I am really worried and stressed, depressed and regret it now. Can you guide me on this? I have no symptoms so far for bacterial infection expect feels some burning at urethra during Pee. But it last for some time only. I asked my partner also about she had previous history of any STDs or any serious disease but she said she is fine and ok. considering all possibilities please guide me. I am very much in stress and have lot of anxiety. Thanks

Its now exactly 10 days after my exposure today and I went to clinic and asked for full STD check up. Doctors did some physical exam on my penis and told everything is good. No signs as off now. But yesterday I caught bad throat and experiencing some cough and feeling of fatigue. I also had heat bump on my (a white color spot on my tongue but its healing). Today doctor said that throat is not too bad. it will heal its own and there is no fever. Is there any reason to worry about this?

I also forget to mention one scenario. I did fingering in her vagina and later she licked it and after that she performed oral sex on me which was unprotected. Is the vaginal fluid can cause any harm in this case?

Brief Answer:
Risk for HIV/STD after oral sex is low.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks foe posting your query.

Oral sex carries a low risk for HIV/STD as per CDC, XXXXXXX
Yeast infection and Herpes infection can occur commonly after oral sex than others,if at all. The possibility of other infections are rare.
Earliest manifestations of HIV infection can occur only after 3-4 weeks after the risky exposure in the presence of an infection. So your symptoms occured after 10 days were not related to HIV. White spot tongue, sore throat, burning sensation during peeing can all occur to anybody without STD or HIV. They are such a common symptoms. Close intimacy during sexual contact can transmit ordinary bacterial or viral infections through air born infection.
Fingering, then licking followed by oral sex also has negligible risk for HIV/STD. Organisms causing STD/HIV can not survive outside the body for not more than few seconds. Moreover the enzyme present over the saliva of human being inimical to HIV. So HIV infection can not spread in this way. So you need not worry.
The surest way to rule out HIV is through blood test for HIV after the window period.

thank you doctor. As you said ..
"Earliest manifestations of HIV infection can occur only after 3-4 weeks after the risky exposure in the presence of an infection. So your symptoms occured after 10 days were not related to HIV"

here risky exposure means deep penetration of genital in vagina? In my case it was just a oral exposure.

Also as Risk for HIV/STD after oral sex is low but what if we consider partner as HIV +ve and high viral load. This will apply in this case also??

Brief Answer:
The viral concentration is negligible in saliva to cause an infection.

Detailed Answer:
Hi,
Welcome back.
Risky exposure means anything other than contact with the regular known partner or wife or when the individual considers as risky. So when you asked queries about the risk about the oral contact the it also can be considered as risky even though the risk is low.

Even if the partner is HIV positive and has a high viral load, then also oral sex is less risky than the anal and vagina contact. Saliva dies not contain adequate concentration of virus to cause a new infection unless it is mixed with blood due to some oral pathology. Presence of lysozyme (enzyme) also decreases the HIV concentration of HIV in saliva.

Brief Answer:
The HIV risk is slightly more with deep kissing and lip-bite

Detailed Answer:
Hi, Welcome back,
It is a pleasure to answer your questions.
Deep kissing in the presence of some oral sores or oral disease would definitely increase the risk of HIV, as also, with lip biting. When the lip bite caused bleeding, the risk will be slightly more.
