I had gone through your question and understand your concerns ..

It may help for some months if given properly by a specialist , it is more recommended with knee arthritis than shoulders ,
I recommend you use it once weekly for 4 consecutive weeks then stop it for 4 months with the help of other medications like painkillers like tylenol tabs when needed , hot massaging with topical anti-inflammatory gels , muscle relaxants will be fine .
If medical and physiotherapy fail and that interfere with your daily normal activities then you may need a surgical intervention as your doctor have mentioned .
