Question : Hi Dr RK. I have gastritis for 3 months and was taking 2 pills nexium 40 mg daily. Based on consultation with you, I have scaled down to 20 mg Nexium in morning and 20 mg pepcid in late afternoon.
1) Can I continue 20 mg nexium and 20 mg pepcid combination for several weeks. I do not feel I can reduce from here very soon.
2)Some times I feel 20 mg nexium and 20 mg pepcid is not enough. Is it ok to add another 20 mg pepcid. Is it better to make it 40 mg nexium and 20 mg pepcid.
3)Is there anything else you can suggest. I am controlling my diet nicely.
Thank you.

Hi,
Thanks for getting back to me with more questions.
1. Yes, you can take Nexium 20 mg and Pepcid 20 mg combination for a couple of weeks. You can also see whether you get symptom relief with Pepcid 20 mg twice a day ( morning and night).
2. If you get symptoms in between while taking Nexium 20 + Pepcid 20, you can take TUMS to get symptom relief from that episode.
Or you can take Nexium 20 in the morning and 20 in the night.
3. Stress relief and good sleep is also important in addition to medications and dietary changes.
If there is persistence of symptoms, get tested for Helicobacter pylori infection if not done already.
Avoid alcohol and smoking.
Regards,
Dr.R.K.

1) I will try pepcid 20 mg twice a day. Is it ok to go to three times a day if needed. Is that better than 40 mg nexium daily in terms of side effects.
2)I understand that using higher dosage of PPI like nexium increase risk of covid but pepcid does not do that. Is this correct.
3)When trying pepcid 20 mg twice a day, can I take some tums for symptom relief.
Thanks.

Detailed Answer:
Hi,
1. Take Pepcid only twice a day. The safety profile of Pepcid is better than Nexium
2. Increased risk of covid with PPI itself is mentioned only in a few studies and hence not generally accepted.
3. Yes you can take Tums for symptom relief while on Pepcid.

Dr.R.K.

Should I take sucralfate also one or more times a day. Mt gastritis has healed to a good extent and I wonder if sucralfate( in addition to Pepcid) will do any good.
Thank you.
