Posted on Tue, 27 Dec 2016
Question: i have been diagnosed with ectopic or tubal pregnancy on right fallopian tube having mass of 2cm in longer dimension on 1/12/16. doc gave me a shot of methotrexate (50mg/2ml) on the same evening (6 pm) that is on 1/12. my beta hcg levels were 928 back then and then they took the blood sample on 4/12 morning 6am for which the result was 1400 B-hcg level. hence the doc gave another shot of methotrexate on the same day at 8pm on 4/12. is it alarming ?? i dont want to undergo a surgery.

I understand your concerns.

Following is my reply:

1) This ectopic pregnancy can be treated by methotrexate and you are on right track of treatment.

2) I suggest you continue with the present drugs which will help you in medically treating it without surgery.

3) Please look out for signs of rupture like sudden pain, bleeding which should alarm you to rush to hospital suspecting rupture.

Was it a good decision to give second shot on day 3? What are the chances of bursting my fallopian tube? Do i need to get admited to a hospital for close monitoring? What extra things i need to take care of?

Brief Answer:
No need of admission

Detailed Answer:
Hi,

There is no need to get admitted.

Second shot was worth it as beta hCG values were increasing.

Difficult to predict chances of fallopian tube bursting

Is Leucovorin 50mg required 24hrs after the methotrexate 50mg/2ml shot as the doctor gave this shot after each consecutive methotrexate injectn. After how long the beta hcg level should start falling ? Plz tell tentative details if you cant predict.

Brief Answer:
Yes

Detailed Answer:
Leucovorin is required after methotrexate. This is to reduce side effect.

Beta HCG should atleast stop rising in 4 days.
