Question : My aunt has UTI problem from past few years and she is feeling the similar symptoms today. Last time she had this symptoms is on Feb,2018 and when she consulted urologist, he suggest for urine culture and told her to start Niftas 100mg for 5 days(twice a day) and Citra Soda(thrice daily) till the culture reports are out. Later in culture report no infections found and also her symptoms subsided with above medicine.

Can she follow the same path now? I mean give the urine sample for culture and start taking Niftas and Citra Soda and take further decision based on culture report? Please advise.

Brief Answer:
Better to take appropriate antibiotic as per culture report

Detailed Answer:
Hello

Based on the facts that you have posted your 54 years old aunt has repeated episodes of UTI .

UTI is a very common problem in females at peri menopausal age group and is mostly due to what is called as Urethral stenosis secondary to hormonal changes .

Since bacteria always change their sensitivity pastern the antibiotic which was effective previously may not be sensitive to the said antibiotic now hence It is always advisable to repeat the urine culture and take appropriate antibiotic as per culture report Regards.

Thanks for your response.

I have already given her urine sample for culture today morning and result will come only after 48 hours.

Last time the urologist suggested for culture and while we were waiting for the result, he suggested to start Niftas 100mg and Citra Soda. But later the culture report was not showing any sign of bacterial growth. Also by the time the report came she was feeling better with above medicine.

Considering this, I just thought of checking about taking Niftas and Citra Soda and then changing the medicine based on culture report. Without any medication she should bear the pain till the culture report is out.

Today we got her culture report and it says there is no growth of organisms. Report is attached for your kind reference.

Pattern of her UTI is as follows:

1. She started getting burning sensation from Tuesday(28th, Aug) and she started drinking lot of fluid.
2. Thursday(30th, Aug) she was complaining more pain and burning sensation and may be because of too much fluid intake she vomited few times. No high fever.
3. Friday(31st, Aug), her urine sample is given for culture and then we started her with Niftas and Citra soda.
4. We got her culture report today(2nd, Sep) but no growth of organisms but she still complains of burning sensation and pain around the area but it has reduced a bit when compared to Friday(31st, Aug)

Her last UTI episode was also like this with no growth in culture report.

Kindly suggest on what could be the reason for this and what can be done next?

Thanks,
Subhash

Brief Answer:
Mostly due to inflammation of bladder secondary to Urethral stenosis

Detailed Answer:
Hello
Thanks for uploading the report and highlighting the facts in detail .

Her symptoms are mostly due to inflammation of bladder secondary to Urethral Stenosis .

Consult qualified Urologist for clinical assessment and get endoscopic examination of bladder (Cystoscopy) with calibration of urethra done .

This will help to empty the bladder completely .However this has to be followed by urethral dilatation at home every day for 3-4 months .This will help to reduce the inflammation and get frequent episodes of similar complains .

Thanks and Regards
