"Bright red" usually means fresh blood. "Black wormy objects" are usually blood clots. Urinary tract infections commonly cause urinary tract bleeding (either microscopic=invisible or macroscopic=visible). This is the most likely explanation for your symptom.

My question is more specific. Previous antibiotics would
show some redness, but the nitrofurantoin turns the whole
bowl a very bright red. Does this drug have a marker
that enhances the color when blood is present?

Blood clots sounds more serious. Are they induced by
the medication itself? Can they get "stuck" in the penis?
Is this something I should bring to the attention of my
internal medicine specialist?

Hi,

I don't believe it's nitrofurantoin that caused your urine to become bright red and I've never read anything about enhancing the color when blood is present. Nitrofurantoin may sometimes cause the urine to become dark brown.

Regarding the clots: small clots will come out with the urine without any consequences at all. Large clots may cause obstruction. If it occurs you're going to feel an urgency to urinate but you won't be able to pass a single drop and your lower abdomen may enlarge (as the bladder accumulates more and more urine). In that case, you would have to visit the ER.

You should bring it to the attention of your doctor indeed, but don't worry too much about it as hematuria is expected during the course of urinary tract infections. A radiological assessment of your urinary tract should be done (if it hasn't been done already).
