Can UTI Cause Headache And Neutropenia?

Question: Okay, my wife had been to ER for UTI like symptoms about 4 hours after epidural steroid injection. Urinalysis showed glucose spillover( +1) but everything else normal . She was put on bactrum and her glucose was monitored by D-stick until she saw her primary doctor 2 days later. Her glucose was normal at fasting, 2 hours post meal, and at random checks. Primary attributed the glucose in urine to the Epidural steroid injection.
She was sent to gyno same day and switched antibiotic to macrobid. she remained symptomatic and noticed a slight discharge and had temprature of 99.4F with slight back pain developing yesterday morning.
She returned to ER last night where they ran full blood panel and urinalysis. Urinalysis showed bacteria but everything else was WNL. Blood work was all WNL including bun creatine and GFR. White count was at 4000.
She has mild non-specific neutropenia with her white count normally around 3000 but it always rises for a time when she gets a steroid injection.
She woke this morning with sore shoulders headache on right side of head and slightly n that is tender when massaged and she is slightly nauseous but eating fine.
She hasnt had any more fever and UTI symptoms have gotten better.
She is now concerned about headache and if complications from injection could be the cause of headache. Sitting, laying or standing does not increase headache.
Shes concerned that neutropenia might mask infection and if blood work would show any type of infection being that she has neutropenia.
Its been 6 days since injection and injection site looks normal(not even visible), no fever.
After all these DR and ER visits she doesnt want to return but is stressed out given her neutropenia.
Her overall impression is good. Shes going about her day pretty normal, just concerned and slightly uncomfortable.

Thanks for the query

Infection especially a severe uti can lead to neutropenia which is transient and recovers on its own. The headache could be because of the epidural injection which settles with good hydration.

Given the nature of her headache and the fact that there are no other symptoms, I will not relate this to epidural injection complication.

Here is how she should go about it

1. Let her take adequate fluid, about 3 liters of water per day
2. watch out for fever and recurrence of burning micuturarion
3. please repeat a complete blood count, neutropenia usually recovers after successful antibiotic treatment
4. I suggest you to send for a urine culture and to get a scan of the pelvis as well

So if she has been diagnosed since 2007 with non-specific neutropenia, if she has an infection will her white count show it or will her pre-existing condition of low white count make her numbers appear normal? This is where we are unsure as these labs are normally used to better determine infection.
Though shes been neutropenic since 2007 she has thankfully remained very healthy over the last 9 years so we arent familiar with how to be vigilant about preventing a possible minor infection from becoming advanced.
Our current concerns of course being a lower UTI moving up to an upper UTI or kidney infection or a complication from the epidural injection causing any type of infection to spinal or surrounding tissue like menengitis.
Right now we focus on her body temprature and make sure she doesnt exhibit more telling symptoms to gauge how shes doing.
So with a moderate headache and intermitent low grade fevers it gets confusing.
Currently she is feeling better with UTI symptoms and no more back pain or fever. She does still have a headache, not sure why
Thanks for any info you can provide as its Christmas and other than ER, local Dr info isnt an option..

1. Decline in neutrophils from previous stable values indicates an infection of viral eitiology, an increase in neutrophils tells us that the infection is of bacterial or fungal etiology

2. One can prevent infections by keeping the hands clean, keeping a distance from people who have an ongoing infection and by avoiding crowded places. Eating clean food with neatly cleaned ingredients can prevent gastro intestinal infections

3. UTI does not lead to spinal infection unless it gets disseminated through blood. In such cases the patients will be very sick and toxic

4. Moderate headache and intermittent low grade fever points towards a viral infection

5. People usually exhibit mild headaches when there is an ongoing infection. Please get it evaluated soon if it does not subside or becomes worse
