Question: How can I switch from xarelto to warfarin. Switch is necessary due to blooding.

Regarding your concern, I would explain that the effects of xarelto are insignificant after 24 hours of stopping it.

So, if you need to switch from xarelto to warfarin, you can do this easily, by taking warfarin instead of xarelto.

I would recommend performing an INR test before making this change of therapy.

After starting warfarin, the dose should be regulated in the next days based on the INR level: you can take 5mg the first day, then 3mg the second day and then perform INR level after two days.

In the first week of starting warfarin INR levels should be performed every 2-3 days, in order to adjust the dose of warfarin.

Usually after the first week of starting warfarin, you should decide with your doctor a usual daily dose of warfarin based on your INR levels.

You should discuss with your doctor on the above issues.
