Question: Thank you Dr. Bishnoi for your follow-up e-mail.
I will be 85 in November. I looked up the drugs that you suggested as a possibility, and because of my age decided against them. However, I did like your idea of killing the troublesome nerve. I have no idea of how someone could locate and kill the nerve unless they have a procedure for exploring the site of the incision.

I can understand your concern. You right that you should avoid painkiller drugs. Because at 85 years age, these medication can cause impaired renal and liver function. However you can take these medication in low dose on intermittent basis.

Regarding nerve lysis, it is a good option. You are worried about how would doctor localise it?

It can be localised with your pain history, normal nerve anatomy, pain site and nerve conduction studies.
Also during procedure, the patient is kept awake. When doctor destroys the nerve with alcohol/phenol/steroid, he/she asks the patient about pain relief. If there is relief, that means right nerve has been hit.
So thats how we do the nerve lysis. You don't worry.
