Question: Dear Dr, Could you please help us interpret the blood test results bellow? Test was done on 11th of March and menstruation was 25-28th of February Reson for the blood test was to check the thyroid hormones and also to check hormonal disorders since patiens has been experiencing excessive hair growth and irregular menses Test Res. Ref interval. Unit S-TSH (ModeE) 1,2 0,3-4,2 mE/L S-T4, free (ModE) 12 12-22 pmol/L S-T3, free (ModE) 6,3 3,1-6,8 pmol/L S-Estradiol 259 follicle phase 17; post menopause <3,0 S-Testosterone 2,1 <2,7 nmol/L S-SHBG 17 Women fertile: 29-95 nmol/L. Women meno pausse: 25-100 nmol/L S-Testosterone bioact 1,3 <0,86 nmol/L What can be said about the results? Coud there be a dissease behind the results? How could the levels naturally or through medication be stabilised? Sorry, two tests were left out by mistake, here they are: S-Prolactin 166 71-567 mIE/L S-Progesterone 3,6 follicle phase 17; post menopause <3,0 Regards

Your SHBG - Sex hormone-binding globulin levels are decreased. SHBG levels are decreased by androgens, anabolic steroids, polycystic ovary syndrome, hypothyroidism, diabetes, obesity, Cushing's syndrome, and acromegaly. In your case since there is an increase in hair growth and irregular periods this is likely to be due to androgen excess- can be related to polycystic ovaries. Hi Dr, Thank you for your answer. Seems there was one mistake regarding the S-Estradiol results. Here is the correct one: S-Estradiol 259, follicle phase <600; mid cycle phase 300-2000; luteal phase 300-1000 Could you please comment on that? Also, what are your thoughts about the following: S-Testosterone bioact 1,3 <0,86 nmol/L Seems like out of range but what conclusion can be drawn from all of this? Best regards

Brief Answer: Detailed below. Detailed Answer: Hello. Thanks for writing again. The serum estradiol levels seem to be slightly lower as your blood sample was taken on day 14- during ovulatory phase. This can be taken care of by estrogen supplementation. Usually these levels vary between 15-350 pg/ml during the menstrual cycle. Highest levels are seen during ovulation. The mild elevation of S. testosterone levels are due to decrease in SHBG. This is also seen in PCOD. It can be treated with the help of medicines. Hope my answer is helpful. Are there any natural supplements or advices you would suggest in order to improve estrogen levels and/or treat PCOD/increase SHBG? Also, would you recommend to stop using the progesterone cream or continue with it? Regards

Brief Answer: Detailed below. Detailed Answer: Hello. In PCOD, the treatment is mainly medical treatment with Oral contraceptive pills for some time which will make your hormonal disturbance improve. But most important is exercising to lose weight and becoming fit. If exercise is not done then the hormonal imbalance will still be there. Proper diet consisting of low carbohydrates, low fat diet with plenty of fresh fruits and green leafy vegetables is needed. Drink plenty of water. Do daily brisk walking for 40 minutes. Avoid junk food, pastries, colas. Balance low GI carbohydrates with adequate protein. Include phytoestrogens in your diet. Buy organic foods whenever possible. Reduce your intake of saturated fats. Limit or avoid dairy. Drink two litres of filtered water daily. Avoid additives, preservatives and food chemicals such as artificial sweeteners. Avoid caffeine and alcohol. Avoid sugar on its own and hidden in foods. Include essential fatty acids in your diet. Herbs like chaste berry, milk thistle and dandelion root are also helpful along with acupuncture. You can continue using progesterone cream as recommended by your gynecologist. Regards.
