Question : Hello,

I am currently pregnant with twins and due to give birth through elective c-section in a weeks time (at 34 weeks).

This is my first pregnancy, and unfortunately, have been reading, and have been told of too many stories (mostly not plesant).

One of my concerns is with anesthesia during the procedure. Two of my friends have described to me of their encounters with not being able to breathe properly during the c-section.

My issue is that, currently I am not able to lay flat on my back at all without becoming breathless and having to adjust myself. When sleeping, I lie to my side and have pillows propped at an 45degree angle to lay my head.

I have been previously diagnosed by an ENT specialist of some curvature in my nose that is inhibiting air flow, however I have not have surgery to fix this.

I have recently also been prescribed Labetalol in my pregnancy to control my blood pressure. And since commencing to take this, have felt more uncomfortable breathing, and the obstetrician has advised that this medicine can narrow the airways.

I am petified of losing consciousness during the c-section.

I kindly seek your advice to my following question, so as to give me some reassurance:
** How will the c-section be conducted if I cannot lay flat on my back? What medical aids/medicine will be given so that I can breathe comfortably given my situation?

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

Brief Answer:
Higher concentration of oxygen can be given.

During a C-section, oxygen saturation and vital parameters like pulse, blood pressure etc are regularly monitored. To facilitate the breathing, high concentration of oxygen is given to inhale. You will have to lie on your back for the surgery. Labetolol can be omitted for 1-2 days prior to the surgery in case the blood pressure is well controlled.
The surgery is done under epidural anesthesia and you will not lose consciousness during the procedure. The anaesthetist will closely monitor the procedure and you need not worry.

Hope I have answered your query. Let me know if I can assist you further.
