Yes, scabies can affect any part of the skin including that of the ears.

Yes, it can cause a bad ear ache if it is severe enough to lead to inflammation.

Yes, it is contagious--if you scratch and then touch other parts of your body, you can spread the infection. Additionally your contaminated linen if used by others can give them the infection.
