There are many possibilities why bronchomalacia happens.
Preterm birth, chronic lung disease, positive pressure ventilation and hypocalcemia have been associated with bronchomalacia.
Coming to your query
A. Early inducement of birth for medical reasons can result in bronchomalacia because of preterm nature being a risk factor as muscles in the respiratory passages wouldn't be fully developed.
B. When infant is born blue it means it has hypoxia
The breathing difficulties could be either cause for bronchomalacia esp if positive pressure ventilation was given Or it could be the reason for the breathing difficulty itself.
I hope you understand that bronchomalacia can be both a cause for breathing difficulty and poor muscle tone as well as it can develop due to breathing difficulty and low muscle tone.
It is difficult to say which came first.
