If a person have some chronic muscle pain in the body, can it affect levels of testerone?I have had low levels before around 11-12.
Its more like a myalgi pain in the body that have been lasted for 10 years due some stress and working shift during night.
Question for this, i am having planning to start a new relationship

Well looking into your problem I can tell you that the chronic pain and myalgias you are suffering from can be due to low testosterone levels. Vice versa its not possible generally. So that may be the root cause of your fatigue. Also have you got any muscle enzymes done to see for muscle activity. Vitamim D deficiency can also give rise to such symptoms but you are already on supplements. Any other issue please feel free to ask.

as it affects my private sex life I can feel tired in muscles to engage in a upcoming relationship and sometimes feel even chronic pain in penis (near Urethra area).
Should I have appointments for check up?
As it also symptoms for possibly having fibromyalgia?

If your symptoms persist after testosterone replacement then we can think of Fibromyalgia but at present we have an underlying cause. Also I think you should see a urologist for above mentioned problems in case you feel any difficulty in your personal life. Chronic penile pain is unlikely in such cases.
