Question: I have been having issues with constant burning with and without urination. My urologist says that it is interstitial cystitis but I do not think it is.

Brief Answer:
Where is the burning sensation?

Detailed Answer:

Hi,

Where and when do you experience this burning sensation? What alleviates and what provokes the pain? Have you had a urinalysis or a cystoscopy?

Best wishes.

I experience the burning with and without urination. It feels like a really bad UTI but I do not have an infection. The only thing that alleviates the pain is icing the area and sometimes taking pain medicine and pyridium helps a little bit but not a lot. I have had a urinalysis which shows blood in my urine but no infection. I also had a renal ultrasound which came back fine. I have had a cystoscopy which showed some inflammation.

Brief Answer:
Are you currently receiving treatment?

Detailed Answer:

Hi,

Are you currently undergoing symptomatic treatment for interstitial cystitis? e.g. round the clock analgesics, tricyclic antidepressants?

Also, have you had urodynamic measurements or a biopsy during cystoscopy?

Regards

I have tried pyridium which only takes the edge off slightly and does not last long. I am allergic to amitryptaline which I think is the antidepressant usually used. I recently started taking methenamine which has not helped and has made me have headaches and shortness of breath. I do not know about the measurements but I think they did a biopsy.

What are urodynamics?

Brief Answer:
I suggest second opinion and urodynamics

Detailed Answer:

Hi,

It is important to know what the biopsies showed - and if there were macroscopic disease found at cystoscopy. Second, I think that it is relevant to get a second opinion. Your urologist may be right that it is interstitial cystitis, but it is (usually) a diagnosis of exclusion. Urodynamics should be performed to rule out overactive bladder.

Brief Answer:
A way to determine the motor function of your bladder

Detailed Answer:
Hi again,

Urodynamics are a way of determining the motor function of your bladder. It involves measures of voiding volume and speed, and it can help diagnose overactive bladder, flaccid bladder, urethral obstruction, etc.

Hope this helps.

How are those conducted? Do those problems have symptoms of burning as well?
doctor
Answered by Dr. Anders Mark Christensen (6 hours later)
Brief Answer:
That actually depends on several factors.

Detailed Answer:

Hi,

That depends. The burning sensation may arise from an overactive bladder. Flow measurements or urodynamics usually involves voiding in a funnel-shaped apparatus - completely painless.

Hope this helps.

Thank you for the information. Do you have any suggestions on ways to help relieve the burning until I figure out what is going on?

Brief Answer:
NSAIDs

Detailed Answer:

Hi,

If you have tried it, I'd recommend NSAIDs (Acetaminophen) and Aspirin for symptomatic relief.
