Brief Answer:
as explained in details.

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Male/78 - reoccurring rectal abscesses for the past one year - type 2 diabetes - Surgeon says is a cause for an abscess and re-occurrence.

My thoughts:
Diabetes when uncontrolled or not well controlled can certainly be the reason or occurrence and re-occurrence of abscesses anywhere in the body, particularly in the peri-anal/rectal area.

But at the same time, other causes should be ruled out:
Crohns
Tuberculosis and such specific diseases and can be diagnosed by the biopsy from the tract alone and hence should always be done.
Anemia, hypoproteinemia are contributory factors.

Colonoscopy should be done in all such cases
Local hygiene should be well maintained, particularly after surgery.
HbA1c should be kept normal - indicates good maintenance of diabetes.

I hope this answer helps you further to get the reasons for re-occurrences.
