Posted on Mon, 9 Jun 2014
Question: My wife is 27 weeks pregnant. She has conceived normally. But since beginning I am seeing a IVF specialist namely Dr. XXXXXXX Banerjee as she is close to my house. Is it alright to see a IVF specialist instead of a Gynecologic even in case of normal pregnancy? will it create any complication? should I take duel consultation?

Brief Answer:
PRACTICALLY MAKES NO DIFFERENCE

Detailed Answer:
Hello XXXXX
Thanks for writing to us with your health concern.
Most IVF specialists are obstetricians / gynecologists to begin with.
A few directly train in IVF.
If the doctor is a basic MD / MS / DNB in OB - GYN, it practically no difference whether you consult her or another gynaecologist.
Are you aware of her qualifications ?
Can you upload a prescription / letterhead that must be mentioning her credentials, so that I can guide you accordingly ?
All the best
Please feel free to discuss further.

Detailed Answer:
Her credentials are topnotch.
She has worldclass qualifications, and there is no need whatsoever to change your doctor.
Have faith in her, and do not go for dual consultation - it only adds to confusion and anxiety.
All the best for a wonderful journey of parenthood.

Great to know. Pls also tell me in case of cesarean pregnancy dose doctor use stiches these day or they simply use pasting? Which one is modern and better and in what way?

Detailed Answer:
Hi again.
See there are various methods employed by surgeons.
The most primitive one is conventional stitches.
The next one is using totally absorbable stitches placed right under the skin - called subcuticular stitches, this is a wonderful way that leaves almost no scar.
The third and most modern way is using surgical staples or surgical glue ( what you mean by pasting I think ) - this is most expensive, latest and best from cosmetic and pain point of view.
Hope the answer helped you.
Take care.

Detailed Answer:
Why have you decided that you would be having a Caesarean birth for sure ?
If it is done for any reason, then you can ask for either subcuticular stitches / staples if you can afford it.
Generally the operating surgeon takes a call as per the per operative findings, you can discuss your preferences with her at the time.

Pls find the attached prescription, Dr. Has prescribed to take the following injection mentioned at the bottom of the page. Pls tell me what for it is required? Is there any side effect of it And can it be taken in hand or hip, which is preferable? Is it painful like TT? Also in case of acidity in this condition antacid can be taken or not, if yes pls suggest some good antacid.?

Detailed Answer:
Hi again.
That injection is a steroid.
It is generally given to women at the risk of preterm labour to enhance the lung maturity of the baby.
Preterm babies have difficulty in breathing, this injection improves their chances of survival.
I really cannot fathom why it is being prescribed in your case.
Is your wife having any risk of preterm labour ?
There are certain side effects on the baby, but they are rare, and when the baby is at risk of preterm birth, the benefits are much more than the risks.
But if she is not having a risk of preterm labour, it puzzles me why it has been prescribed.
It is not painful, generally even TT is not painful if given properly
Injections in the hips are preferable, due to more fat, pain is less.
Yes, antacids can be taken alongwith it.
A good antacid is Pantop - D.
Take care.
Hope this helped.

Thanks, i also not even sure of the fact that she is prescribing the same for my wife as till date she dosen't having any symtom of preterm labour. But according to the doc she is saying no one will be or can be sure or confirm of this incident of preterm labour/premature baby, so she prescribe this to all her patients irrespective of the risk factor involved of preterm labour. Is this justified explanation of having this injections? Also pls tell me incase of Caesarean birth when will my wife be able to travel as we r planning to go to XXXXXXX also flight or train which will be better medium of travel that time.

Detailed Answer:
There is NO EVIDENCE in administering steroids to all women, irrespective of the risk of preterm labour
While I respect the doctor's decision, it does not have evidence based backing, and I would not advice it to a patient of mine.
IN case of Caesarean birth, your wife is best advised not to travel for atleast 6 weeks after the surgery, once she travels, if it is a short flight, that is better than train travel, and if the flight is long distance, a convenient , hygienic coupe booked in a train is better.

Hi, my wife is having pain in the naval area sometimes in these days. The feeling is kind of stiffness in around the naval area. N i guess she is also suffering from acidity sometimes in these days. Appreciate your advice on these issues.

Brief Answer:

Detailed Answer:
Basically as the third trimester approaches, the uterus greatly grows in size and puts pressure on surrounding organs, thus leading to pain, discomfort and acidity.
Pain around the umbilicus ( naval area ) is common as that is the point at which maximum stretching occurs.
For acidity, follow the below pointers -
1. Have 5 - 6 small meals every 2 - 3 hours instead of only 2 or 3 big meals.
2. Never skip breakfast
3. Have a glass of chilled milk at night before sleeping.
4. Sleep atleast 2 - 3 hours after your last meal.
5. Sleep propped up, with 2 - 3 pillows underneath the head.
6. Avoid spicy, oily, greasy food.
7. Have lots of bland food and fruits and vegetables.
8. Avoid stress and negative thinking.
9. Do not eat outside.
10. You can use an antacid like Pantop D daily, it is quite safe.
Take care.
