Brief Answer:
You should undergo valvotomy

Detailed Answer:
Looking at your report your valve has become quite small and you should undergo valvotomy. It is more for narrow valve than leak. It simple and relatively safe procedure requires one day or at the most two days of hospital stay and sucessful in majority. Once sucessful BMV/PTMC you should be able to carry on all normal activities. Unless complications it doesn't really affect your longevity. But knowing that you are only 32 years there can be need for further procedures in future such as valvotomy or Valve replacement.
You need continue your Penicillin prophylaxis.
