Can I Have Abdominal Hysterectomy Despite Having IBS?

Question: I am scheduled for abdominal hysterectomy, sacrocolpopexy, bladder sling and an anal
sphinctoroplasty. And I was wondering how long I can expect to stay in the hospital. I am also concerned about the fact that I have ibs with both constipation and diarrhoea. In either case I tend to strain with out even thinking and I'm afraid of doing damage after the surgery. My doctor knows about the ibs. Am I just being paranoid? Or are there things that can be done for prevention? I really don't want to cause any complications.

as mentioned you are scheduled for a prolonged surgery with abdominal hysterectomy,sacrocolpopexy,a bladder sling and anal sphincteroplasty, i would like to appraise you that you would be admitted two days prior to the surgery for preop preparation which would include your complete blood profile for basal counts and bowel preparation . bowel preparation should start from the day of admission where you would be given a laxative like PEGLAC to drink so that bowel can be evacuated completely on day 1 followed by a clear fluid diet , the next day, the day prior to the surgery. this is done with an aim to prevent any bowel distension and loops of intestine to interfere with the surgery. your hospital stay post surgery should be a minimum of 4- 5 days depending on the recovery you would exhibit.
day1 & 2 post op= you would be kept nil orally and placed on IV fluids and parenteral antibiotics and pain relief with epidural anaesthesia and injectable analgesics along with antacids for gastric protection.
they would watch for your peristalsis to return and how soon you pass flatus ( which indicates recovery from bowel paralysis, which is a common feature post abdominal urgery and manipulation of bowel loops).
day 3= if your bowel sounds have reappeared and you have passed flatus , then they would switch to sips of clear fluids ( tea/coffee/soups) and encourage you to ambulate. pain relief would be shifted from epidural to injectables.
day 4= depending on the counts and blood profile and your electrolyte state, they may consider parenteral feeding if you are unable to tolerate orally as nausea and vomiting may exist. ambulation would be further encouraged and you would be asked to walk frequently during the day with support. perineal care would be offered by the nurse. if you show improvement by day 4-day5.,then after the diet is established and you are tolerating well the clear fluids and light diet ( bread/coffee/soups/ boiled cereals) and you have passed stools comfortably , then they may consider your discharge depending on your progress.
if these milestones are achieved later or are delayed accordingly your stay shall be prolonged.
considering IBS, it should not be a deterrent for your surgery or post op care . the care would be based on your different parameters and how you respond to treatment.
antibiotics are usually continued for 3-5 days depending on the recovery by the patient.
abdominal would dressing shall be removed on the 3rd or the 5th day depending on their protocol. as far as suture removal is concerned , its a practice to apply absorbable sutures and give a cosmetically friendly incision line with subcuticular stitching.
you would have to restrain /refrain yourself from lifting and weights for next 3 months.
so on the whole 5-7 days should be your post op depending on your recovery.
there is actually no need to be worried , you should be expert hands. just that a will on your part would be required to get well at the earliest. pain could be a deterrent for you and would prove heavy on your will power but that you have to mentally prepare yourself even though pain killers would be given but 100 % pain relief is not achieved once you would be off epidural .
just follow the advise of your treating consultant and everything should go just fine. i wish you a speedy recovery. just take the surgery with a positive frame of mind with an intent that all your problems would be removed after the surgery and you would arrive a healed soul.

Thank-you for all the information. It is a big help and takes alot off my mind.I will do the best I can to get through it keeping your note in mind. Thank you oh so kindly. I am trying to remember that I went through something similar when I had my daughter. A botched ipisiotomy and the use of forceps left me with 50 stitches. Surely this can't be worse....can it? Again thanks. :)
