It is possible if you have cough. During the cough the pressure in the abdominal wall increases and if you are prone to have inguinal hernia, during the cough the intestine or fatty tissue from the abdominal wall could have been pushed to the scrotum. If so, you should be able to feel movable masses in the scrotum which do cause some pain especially when coughing.

Other possible reasons of scrotum swelling (unrelated to pneumonia) are testicle inflammation, epidydimis, hydrocele, varicocele. Sometimes due to lack of albumin in the blood (caused by kidneys disease for example) can cause scrotum swelling es well. However in such case, swelling should be also present on the legs.
