Can Psoriasis Be Misdiagnosed As Mycosis Fungoides From A Biopsy Test?

Question: I have been recently diagnosed with Mycosis Fungoides from a biopsy taken from my belly which was originally diagnosed as Psoriasis or Exzema. This was a diagnosis as a result of a biopsy being sent back east. I was given a cream Clobetasol 0.05 to apply twice a day and only for two weeks on and two weeks off. I have only the one patch. I have had this patch for 1 &1/2 years with no spreading. I wish to know if it is usual for a misdiagnosis of this cancer and what path I might take is determining that. My doctor told me that she asked the lab to check is second time with the same result. I just want to know if this is a condition that can be easily misdiagnosed.

Thanks for your query.

This is a rare disorder and mimics many other common skin disorders. Hence many a times the diagnosis is missed and treatment is given for other disorders. In your case, the diagnosis has now been made and fortunately it is localized to one area. So result of treatment is usually good.

Now you have the doubt that it is something else and wrongly diagnosed as mycosis fungoides. This can also happen. A review by an expert hematopathologist is essential to determine that we are dealing with this only.

The same lab can check it again but more important would be to get the slides and block of biopsy reviewed at another center which specializes in this. They can do special tests on biopsy to confirm the diagnosis.

In my first e-mail I put the medicine I was prescribed as "Clobetasol 0.05% cream to be taken twice a day. It is much stronger than the first medicine I was prescribed and wonder if it could cause terrible itching. I also wanted to ask if with one patch in phase one of this disease should a person take any kind of internal medicine. I am presently using only the topical medicine.

clobetasol unlikely to cause terrible itching

Detailed Answer:
Yes clobetadol is strong but probably not causing the itch. For itch you can take oral antihistaminics. That should suffice.

And for only one patch, no internal medicine is required. One topical should be fine.
