Can Reducing Solian Dosage Affect Treatment For Psychotic Behavior?

Question: Dear dr. Sheppe,
My name is XXXXXXX i talked with my psychiatrist some days ago and he decided to wait another week with Solian 50 mg, so, a whole month with this dosage instead of 100 mg. My fixation is not gone at all even if there is an improvment of 70 % 80 % and i think now that this improvment is very stable. I have two questions: first, going back to 100 mg after one month, which factor(s) can help to avoid going back to the worsening of fixation? A quick work of Solian in 5ht7 or 5ht2b, more quickly than dopaminergic activity, or something else, including competition with other meds? I fear going back to worsening and you told me in your last letter that this can maybe (even if not surely) happen. In this case, would you recomend me at least for a while another antipsychotic and if so, which one?

First, I want to congratulate you on the improvement you have seen. 70 or 80% is not 100%, but it is a large improvement, and this is wonderful! It is thanks to your determination, intellect and commitment to getting the best care that has contributed to this improvement, so you should be congratulated.

If you do indeed want to try to increase SOLIAN back to 100mg in order to attempt to fully eradicate your symptoms, I can think of only one thing that will ease your 5HT7/5HT2B receptors through this transition: first try an intermediate dose of 75mg. You can split the 50mg pills to achieve this.

It appears your receptors are very sensitive to changes, so I would recommend this intermediate dose, as perhaps this will have the best effect without returning to the 100mg dose that carried negative side effects with it.

If you are interested in another antipsychotic, one I have had in mind for you for a long time is Abilify (ARIPIPRAZOLE). It is expensive, but generally has very few side effects, and similar to Solian is a D2 partial agonist, which would act more gently on your sensitive receptors. Do some research on it and let me know what you think.

Dear Dr. Sheppe,
Thank you for your answer. I just have another question: how long should i take 75 mg until i go up to 100 mg of Solian? I say this because from a psychological point of view 100 mg is the best dose for me (against anxiety) and i was on it during four years and a half without any fixation. I mean that 100 mg not always carried negative effects, this is a very important point.
There is some kind of explanation for having this kind of sensitivity in receptors?

I would recommend 2 weeks on 75mg of SOLIAN . This would allow the medication to reach steady state and then give you some time to experience it at full effect and judge to see if it represents an improvement. If you're tolerating it well, you can then increase to 100mg.

There are small genetic differences from person to person in metabolism of drugs and arrangements of receptors that make people more or less sensitive to medications. It's all related to genetics!
