Hi,
Previously there was limitation to the number of vaccines given at a time. Now you may take all the above listed vaccines together. However it should be given individually and not using one single prick. Therefore I might recommend taking it at different dates if you are not comfortable.
These vaccines are available at all hospitals (multidisciplinary hospitals). Please visit one closest to your location. I am not aware of cost of these vaccines but I guess it would cost you about Rs 5000 - 7500 INR.
Expect MMR vaccines, you will require more than one dose of vaccines for complete protection. Therefore it will be a good suggestion to try and recollect vaccines taken earlier. That being said, taking it again will not harm you.
