Question: Can you please make sense of my results or what my next steps should be? My OBGYN suspected I had PCOS. I had a transvaginal ultrasound and they did not find cysts. They told me to stay on my birth control pill and wait until the end of my placebo week to take a hormonal blood test.

Here are the results as of November 25, 2022:

FSH: 2.3 mIU/mL
LH: 0.2 L

Testosterone, Free: 0.6 pg/mL
Testosterone, TOTAL, MS: 21 ng/dL

Prolactin: 15.6 ng/mL

Hemoglobin A1c: 5.4

My thyroid has also been tested since 2017:

2017: 2.76
2019: 2.32
2020: 2.78

PCOS is a diagnosis of exclucison

Detailed Answer:
Good day Ms XXXXXXX

Diagnosis of PCOS is not just based on ultrasound findings.
You may have normal appearing Ovaries, but if you have high levels of male hormone and irregular periods, it still can be considered as PCOS

From the available results, i can see that your LH is low which could be due to use of OC pills. ( are you still on OC pills?)

Testosterone total is high. Was the test done in the second day or third day of the periods? ( and whilst on OC pill?)
If so, it is a high level and warrant further evaluation with detailed scan including MRI scan of ovary and adrenal glands.

There are few other tests which are needed before we decide further.
17 hydroxyprogesterone, DHEA-S
Please do these tests on the second or third day of the periods before 9 Am.
However it shouldnt be done when you are using OC pill
