Hello,
The presence of fluid in pouch of in the usg pelvis indicates pelvic infection. If clinically you have pain in complete lower abdomen including bilateral iliac fossa and suprapubic region with occasional fever , then its likely that you have PID.

I feel a frequent need to urinate and frequent abdominal cramps especially on my left side & sometimes feeling crampy down there as well. If it is PID, do you think this is a severe case?? Will antibiotics be able to cure it or will I most likely need to be hospitalized? I’m also very worried about not being able to conceive children..

Hello,
It doesn't look like a severe case as such but you shall need a high vaginal swab culture and also a pap smear done to rule out cervicitis and mixed vaginosis as well which are pre-cursors to Pelvic infection. They have to be treated along with urinary tract infection and i am sure they can be treated with a 2 weeks course of antibiotics and urine alkalinizers and antispasmodics.
