Question: My Husband has a issue while urinating, the steam is not steady, on and off. He just had a PSA test and it was 9...The Dr. performed a rectal exam which was normal. Could the PSA score of 9 mean prostate cancer? He is 58 and has takes Metformin for high blood sugar levels.

Brief Answer:
Needs Prostate Biopsy to rule out Prostate cancer.

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that your husband who is 58 years old has urinary problems and his Serum PSA is reported to be 9.

Digital rectal examination done by your Dr is not confirmative test to rule out cancer of the prostate.

Raised PSA can be due to one of the following diseases.
1) Benign Enlargement of Prostate
2) Infection of Prostate.
3) Cancer of the prostate.

The only test to confirm or rule out cancer of the prostate is to get his multi core Prostate Biopsy done.

His raised PSA can not be due to Metformin.

Hope I have answered your query ,please feel free to ask if you have more questions ,
I shall be happy to help you
Thanks and Regards.

Wouldn't the enlarged prostate been picked up by the Digital rectal exam? Is a score of 9 considered high?

Brief Answer:
Value of more than 4 is considered abnormal

Detailed Answer:
Hello
Thanks for follow up.
PSA value above 4 is always considered to be due to cancer of the prostate unless proved otherwise.

PSA is the basic test done for screening of cancer of prostate. It is the normal protocol to go for Prostate Biopsy if the PSA is above 4ng/ml.

Thanks and Regards.
