Question : Namaste sir,
i regularly check my lipids,FBS,PPBS,HBA1c.
i prepared a chart of the same which i will upload.
you can observe for last 2 to 3 years(2015,2016,2017), the values are in acceptable limits. But suddenly,since december 2017 there are spikes in my FBS and HBA1c where LIPIDS are still same .
I didnt change my diet much.
my breakfast is millet based upma (1n1/2 cup),lunch contains 2cups of brownrice with dal and curry and curd,evening snack 1cup papayya pieces,dinner is 3 chapathis with curry and curd. all items made with less oil. BUT i have sweet tooth.So, i eat regularly sweets,chocolates,bakery cakes n biscuits,icecreams(not in large quantities but one or two pieces daily).
With such input, i maintained my levels which u can see for last 3 years.

But, all of a sudden my FBS and HBA1C hiked from december 2017.
my weight is now 97KG and height is 5feet5inches( i know i m obese.)( i reduced weight of 8 to 9kgs in 2015 and for a short period i was 86 to 87 kg from last quarter of 2015 to 1st quarter of 2016.Then agained gained weight of 4kg in 2016 and another 4kg in 2017. REASON :sweets)
I didnt change food etc.
The only change i made is EXERCISE.
I decided to start exercising and started in december 2017 where i tried brisk walk.and then joined gym in jan2018 and regularly exercising for last 45 days.
i use a HR watch to monitor my BPM.
On treadmill, I walk at 6KMPH for 15minutes at 128 to 130 BPM. for every few minutes like in 3rd minute,8th minute,12th minute i increase the speed to 9KMPH and RUN for 45sec(where my BPM rises from 128 to 138 to 144 to 155...sometimes to 160)...i dont like to cross 160 BPM since i almost reached to 85% of my maximum heart rate.So, i reduce the speed to 5.5KMPH and walk again such that my BPM returns back to 128BPM....THEN i do eleiptical trainer for 10minutes at 134 BPM....THEN for few minutes i do noukasan and plank etc.
This kind of exercise i never did in my life. I started and doing it from december2017. And the spikes in FBS and HBA1C are also from december 2017 only.
What is your opinion sir? Is it because of exercise, my Blood sugar is rising and there is no enough insulin to compensate it.
(NOT only the reports i uploaded but i regularly check my FBS at home using glucometer which always shows readings like 116,117,110,120 etc for last few years. But suddenly i see 147,145 etc)
please tell me sir

i attached all previous reports for years 2016 and 2017.
But i wrote the values on 3 papers--one contains list of FBS and PPBS values(and also Random BS values),another contains list of HBA1C values, and another contains list of LIPID values.
They are the 1st 3 jpg files.

I am sorry but I did not find anything surprising. You are doing everything in terms of diet and exercise, but at the same time, you are not doing what should have been the first step, give up on sweets.

You are significantly overweight, but you are not accepting the problem. You must start on a weight loss plan, and now of course, diabetes management plan.

Exercise never causes high sugars, that kind of hypothesis does not exist. Please start taking a wise decision, give up on everything that is sweet, try for a week, continue exercise, recheck your sugars after a week and let me know.

OK madam,
I will stop eating sugary foods.
I didn't eat any sweets for last 3days (after seeing these reports).And I didn't face any sugar cravings. So I can manage without eating them.
Can I eat fruits as a snack. Please tell me which fruits I can take n which I cannot?
I will continue to exercise. But sometimes I don't get time in morning and do in evening time. Such evening exercise will be helpful or not. I read somewhere that evening exercise effectiveness will last for next few hours and likely to reduce Morning FBS

Brilliant! I know you can do it.

Yes, fruits can be taken. Very sweet fruit like grapes, cheeku, pomegranate, litchi and custard apple are to be avoided. The rest can be taken in moderation i.e. 100 gm at a time, not coupled with more food or more calories.

Exercise done at any time is good and adds to your glucose uptake independent of insulin. Please do it whenever you have time. Recheck your fasting and post meal sugars in a week's time and let me know. If it is still not controlled or even if controlled, we can add some medicines for weight loss.

Namasthe madam,
I followed your advice and didnot eat a single sweet for last 20 days(except on one day where i ate choco biscuits).
I tested my blood sugar after 20 days....FBS is 132 and PPBS is 143 and triglycerides is 267.
i attached the reports.
i am daily exercising for last 3 months but increased intensity for last 20 days as given below:
*20 minutes streching
*15minutes on Elliptical trainer maintaining my heart beat between 128BPM to 135BPM
*15minitues on Treadmill walking at 6KMPH at 128BPM and running at 9KMPH for 3times of 1minute duration while raising my heart rate to 155BPM.
*20 minutes of exercises like noukasan,crunches,plank and other types while lying.

My diet as follows:
1)I drink 2 glasses of warm water immediately as i wakeup and drink another 1 glass of warm water after 30minutes for proper bowl movement.
2)I eat tulsi leaves and 1 tsp of fenugreek seeds soaked overnight.

3)BREAKFAST at 8.30AM: 1 tsp of chiaseeds soaked overnight + 2 almonds + 2 wallnuts +few seeds like pumpkin,muskmelon + 1 tsp of flaxseed powder + 1 tsp of hummus + few garlic cloves + 2 pearl millet(Bajra) Roti OR 2 jowar Roti OR 1and half cup foxtail and little millet upma

4)LUNCH at 1PM: 2 cups of Brown Rice+ 1 cup green leaf dal+ 1 cup veg curry+1 cup curd mixed with a pinch of turmetic,cumin,ajwain,cinnamon,black pepper.

5)EVENING SNACK at 5.30PM: 1 cup of papayya pieces

6)DINNER at 8.30PM: 2 chapatis + 1cup curd mixed with green leaves +1 tsp chutney.

Tell me madam what is your opinion.

The first 3 jpg files attached are about my BS values for last 3 years whcih i added while consulting you for 1st time.
the 4th jpg file is my current BS value.
The next jpgs contain my Breakfast,Lunch and dinner.
So,please go through and let me know about any changes i need to make in exercise and diet.
I have few questions for you which i will ask in this follow-up.



and one more thing...
my weight reduced from 97+ kg to 95+kg because of exercises i did for last 3 months.

Hello,

That is superb! The motivation and dedication, both are marvelous. However, there are still few things.

1. How long can you follow this diet?
2. How long can exercise be followed for this duration and intensity?
3. Fasting is still high.
4. TG will take time to improve, may be 12 weeks.

Therefore, the better path would be to start with some oral medication E.g., tab Metformin twice a day, before breakfast and before dinner. Continue 45 minutes of vigorous exercise and re-check your blood glucose after a week and let me know.

Are you suggesting metformin as weightloss medicine.....I didn't observe my bedtime glucose.......if I start using it then is it possible to stop once my blood sugar come into normal range

How will Metformin work for weight loss?

It is true that Metformin will support weight loss by
1. decreasing appetite
2. decreasing insulin resistance
3. decreasing adipogenesis

At the same time, it will help you control your fasting hyperglycemia as well.

Yes, it is definitely possible to withdraw Metformin once the weight and sugars are controlled.

OK madam,
I will start the metformin treatment after few days( 1 to 2 weeks). Its not that I don't want to follow your advice.
I want to test my blood sugar values whole day at 2 hour intervals on 3 days for knowing how is the pattern.means I want to know my lunch time,evening,bedtime and very early morning( like 3AM)blood glucose values. I purchased glucocard glucometer. I will do this in 1st week of April and then will go through all other tests at diagnostics centre. I will inform you and then I will start medication.
I already regularly testing fasting BS at home which is at 135+ to140+ range and after doing exercise for 45mins it is coming down to 120+ to 125+.
Meanwhile please clear few more doubts:
1)I normally eat my dinner of 2 chapathis with green leaf curry or rasam,curd at 9 pm. And sleep at 10pm. Do you suggest Any other protein packed dinner or my regular dinner is ok? Shall I eat egg whites as dinner to reduce morning highs
2) any suggestion about Apple cider vinegar to reduce fasting hyperglycemia
3)shall I start eating fish and paneer in lunch or dinner for protein diet
4) what if I eat 2 apples at 6pm and 1cup papaya at 9pm instead of regular dinner?
5) shall I start drinking 1 glass wine at dinner time to reduce fasting hyperglycemia.
( I read all such advices in forums online..mso asking)

Hello,

If you want to monitor your every minute sugar instead of pricking so many times and disturbing your sleep, you can go for AGP. Any doctor would do that for you. It is economical and a better option. It records 72 readings a day for 14 days.

As far as diet is concerned, you need at least 2 hours gap between dinner and sleeping.

The variation in diet alone might not affect fasting hyperglycemia because there are many more underlying patho physiologies going on.
