Could Vaginal Fluids Transmit Through Clothing To Cause HIV?

Question: Can HIV be transmitted through soaked underwear?
Dear Dr.,

Three weeks I visited a massage house, I was completely naked but the lady had her cloths on but with very short skirt, she sat with her underwear on my bare penis, only underwear no other cloths and I am not sure if there was any vaginal secretion or not.

Because of her body moves on my penis I asked her to move then after 3-5 seconds, I climaxed in my own hand.

Now the questions:

1- Assuming that she is HIV+, can she transfer the virus to me if there was any vaginal secretion on her underwear to my bare penis?
2- Can the HIV virus exist on my penis till Iclimaxed then go inside my body through my semen?
3- Do I need to do any kind of test?

I am Dr Bharatesh D Basti answering your query. I have gone through the details.

The chances of transmission of HIV by unprotected intercourse with a HIV positive person is 1/200 to 1/1000.It is too low.
There is a cloth/underwear between.So the chances is almost nil.Cloth too acts a barrier. HIV does not survive outside the body for too long. Presuming it too be soaked with vaginal secretions ,there is minor possibilities of transmission but is too low.
She may be negative too.

The chances of transmission of HIV is too low.
You can get her tested or can know her HIV status if possible.
You can get tested for HIV (HIV 1&2 RNA PCR) to allay fears .

Thank you for your input, to understand the chances are almost nil?
Of course, I cannot tell whether her underwear was soaked or not, let us talk the practical case, can this considered as exposure?
To double confirm, what are the needed tests and when I should do them to clear all my doubts?

To be practical the risk exposure is too very low. There is no chance of transmission of HIV through the cloth.
Still to be on safer side, get HIV 1&2 RNA PCR tests now to allay fears. It is >99% sensitive.
Elisa for HIV 1&2 antibodies after 3 months exposure can be done, to be free totally from anxiety.

Thank you for your reply, for sure I will do the tests to clear my mind.

But till then since I am abroad for a work assignment, I will not go back home except after two weeks and the anxiety could kill me.

So please answer me following questions:

1- Should I worry till then or I can feel relaxed?
2- The tests HIV 1&2 RNA PCR tests are enough or still I need to wait for 3 months again to do the antibodies tests?

Results Conclusive after 3 months though PCR and combo are sensitive.

Hiv1&2 PCR after 10 days exposure can be considered conclusive .Some even consider combo test( P24 antigen +HIV 1&2 antibodies test) after 1 month Conclusive.
Though both are very sensitive. It would be better to go for antibodies test(HIV 1&2 antibodies) after 3 months to allay totally fear of HIV.
