Question: Hi, i have red spots on my scalp around the side and back of the head. They are between 0.5cm - 1cm in size. I have 6 red spots in total. They are flat.
I asked about this before on this site and was advised to buy a special type of shampoo. The shampoo did not work and I would therefore like a 2nd opinion as to what it could be and would could help remove the spots. They are not itchy or sore. I am in good health and do not smoke.
Thank you

Brief Answer:
seems like it is folliculitis or contact dermatitis

Detailed Answer:
Hello,

Thanks for posting your query and uploading the pictures. I have read the query and seen the pictures.

Going by your description of the spots - red in colour, the site (as described by you and seen by me in the pictures), their size, the flat nature, non-itching, not sore - it most likely seems like Folliculitis or Contact Dermatitis.

Folliculitis arises when the hair follicle is infected by a micro-organism. This usually becomes painful or sore, if the size of the spots is large and is grossly infected by pus. Your lesions seem to be lesser in size, they are not sore, and they are flat. However, I would still consider Folliculitis as the first diagnosis in your case. Please let me know if you used Razor or blade or any other sharp objects on your scalp recently. Because these commonly cause or spread infection.

Secondly, it could be Contact Dermatitis. It may have been possible that your scalp would have come in contact with any foreign body for which it is showing a reaction. Could you please tell me how long these red spots have been there for, on your scalp? Contact Dermatitis usually occurs suddenly and abruptly, after contact with an allergen or foreign body. It is usually not sore. It does not cause any other major problems as such. Usually, it subsides on its own.

If it is Folliculitis, you would require a course of oral antibiotics to be taken. A combination of Tablet Amoxicillin with clavulanate would be ideal. This is a prescription drug. Also, topical antibiotic ointments like Soframycin or Mupirocin would help. You have to apply the ointment twice daily on the affected areas after gently cleaning them with lukewarm water.

For Contact Dermatitis, you need to remove the foreign body (or the allergen) exposure first. You need to think over this. It could be a hair product that you are using that is causing this, it could be a food allergen, or an external allergen like dust or atmospheric smoke, etc. For this, you can take simple anti-allergen medications like Tablet Chlorpheniramine or Cetirizine orally twice daily for 5 to 7 days for this to subside.

For your information, using too much hot or too cold water on the scalp could also cause your condition.

I would like you to be rest assured that this does not seem like a major scalp problem. It could be resolved sooner.

I hope this helps you.

Please revert to me to be able to help you better with your health concern.

Regards,
Dr Shanthi E

Thank you for your reply. I believe i have had the spots for a year. Perhaps longer. I often get my hair cut very short on the side with clippers. Could that be the cause? Thanks again

Brief Answer:
yes, it could be the cause

Detailed Answer:
Hello XXXXXXX

Thanks for the revert.

Your problem seems to be long standing. Yes, getting your hair cut very short with the clippers could cause your problem. I suggest you leave or cut your hair at a little longer length.

Please take the course of oral antibiotic and use the topical ointment as suggested for 5 to 7 days. After which, I would suggest you to use mild anti-fungal shampoo like Nizoral or that of any other brand which is available over the counter, twice in a week on dry hair. Do not expose your hair to extensive heat or atmospheric pollution.

Please get a blood routine examination (complete blood count), to check if it is normal. (Since your problem is lasting for more than a year now).

Do not worry; you will be fine soon.

Hope this helps. Let me know if you have more doubts or concerns.

Wish you good health, and thank you too. Take care!

Regards,
Dr Shanthi E
