Question: Hi, Could you please advise between Rosuvastatin and TONACT TG 10? I am 69 year old female with history of hypertension for which I am taking Amlong 2.5 mg and Ecosprin 75 mg for over a decade now. I had kidney stones due to which one kidney is only partially functional and had Cholecystectomy around 2008. My weight is constant around 50 KG and I am fairly active with regular walk of around 2KM average on daily basis. During my regular blood test at the start of this year my Cholesterol was: Total: 268, Triglycerides: 197, LDL: 151, and HDL 78. HbA1c was 5.6% within range though just at the top end of reference range. Among electrolytes, Potassium was 5.54 mEq/L (ref range: 3.5 - 5.10), Phosphorus was 4.08 mg/dL (ref range: 2.80 - 4.00). And parameters from Kidney functions Urea was 47.42 mg/dL (ref range: 17.00 - 43.00) and Uric Acid was 7.43 mg/dL (ref range: 2.60 - 6.00), though Creatinine was within range with observed reading of 0.90 mg/dL. Only other two readings: Total Protein: 8.12 g/dL (ref range: 6.40 - 8.10) and Albumin: 4.66 g/dL (ref range: 3.20 - 4.60) were outside of reference range.

With this background, what would be the best choice to manage Cholesterol? And, what does out of range readings suggest?

Hello

I can understand your concern about your health and I would like to tell you that ideally total cholesterol levels should be less than 200mg/dl and Triglycerides should be less than 150 mg/dl. In your case both are high. Your ldl is also high ie above 150 mg/dl ideal is less than 100.

Choice of Atorvastatin/Rosuvastatin depends upon ldl levels as Rosuva offers better control.

You can go for Rosuvastatin+Fenofibrate combination (for high Triglycerides).

Protein and albumins are slightly deranged but your uric acid and potassium levels are high. Avoid protein and potassium rich foods for couple of weeks.

Thank you for getting back, my follow up to this is: should we be worried about “fenofibrate” possible effect on Creatinine? Or, is there a lower dosage of Fenofibrate and Rosuva could be a substitute? If not, then at what interval should we review the outcome?

As your triglycerides are not significantly high you can go with 10mg of Rosuvastatin and 160mg of fenofibrate for a period of one month. Review your lipid profile in fasting state after one month.

Also keep yourself physically active and avoid oily, fried and junk food stuff.
