Question: Dear Doctor,
I am 59 yrs and have diabetes type II since last 14 years and was taking following medicines;

PIOZ 15 (Pioglitazone Hydrochloride), before breakfast
Metformin 850, after breakfast and dinner
PPG 0.3 (Voglibose), with breakfast and dinner

Since last few months sugar level was not under control, hence doctor added Gibtiulio (Empagliflozin) 10mg before breakfast. It seems working but urine now has some smell. The doctor has cautioned me about UTI and suggested to maintain the hygiene.

I am also prescribed Alfusin 10 mg (Cipla) for prostate related issue. I am taking since last three months. Have got some relief. Should I continue it for some more time.

Since last few days doctor has also prescribed Eritel 20 as my BP is lying in the range of 85-90/ 130-140.

Since last one year I have problem of erectile dysfunction (ED). Have desire but can not sustain for a reasonable time.

I would like have your opinion regarding above medicines and medicine for ED.

Hello,

First and foremost, it seems to me that the medicines you are taking for sugars are keeping the sugars under control (after the recommended dose modification).
If you are having a feeling of smell in the urine, I would recommend testing urine routine microscopy, urine culture/sensitivity and urine ketones.
I would recommend letting me know the reports as soon as possible.

As for the erectile dysfunction, this may be developing due to the excess sugars that may be leading to diabetic autonomic neuropathy.
The first and most important step in this is the treatment of the diabetes and strict glucose control. I would suggest testing HbA1C.
For symptomatic treatment, I would suggest trying sildenafil 50 half hour prior to the act. A cardiac check up is advisable prior to initiation of taking of the medicine.

Thanks for the response. I will follow your prescription and will share further test reports.
I would like just to get sure whether the sildenafil 50 as suggested by you can be taken while I am taking Eritel 20 for BP.

How frequently the sildenafil 50 can be taken? Is there any side effect of the medicine?

I noticed smell in urine only after starting Gibtiulio (Empagliflozin) 10mg. Could it be due to the medicine?

I have another query. I take wine/beer 4-5 times in a year only while attending meetings. how much wine/beer is advisable for diabetic persons.? Does the intake of wine/ beer increases or decreases blood sugar level?

Thanks for your response.
I will be answering the specific questions put up by you.
1.     Yes, wine and beer should be avoided. As far as possible. They do increase your sugars and therefore also the HbA1C. So I would ideally recommend not taking wine/beer at all.
2.     Empagliflozin is known to cause yeast infections. This infection can itself cause you to have the smell in the urine. Therefore I had requested the tests.
3.     Sildenafil is to be taken only half hour prior to the act as I mentioned before. It is not to be taken in a regular dose. It does have cardiac side effects. Therefore I said that a cardiac test should be done before starting the medicine.
4.     Sildenafil can be taken with Eritel. Assuming the latter has been started for BP, not a heart problem. In case of a heart problem, sildenafil should be avoided with or without eritel.
I hope this helps clarify your doubts.
