I am reaching out on behalf of my wife, who is currently in her 8th month of pregnancy. We have observed some concerning symptoms and would greatly appreciate your expert advice on the matter. Here are the issues she is currently facing:

a. Gums Bleeding: She experiences bleeding gums during tooth brushing.
b. Swelling in Legs: There is persistent swelling in her legs throughout the day, and it doesn't seem to be reducing.
c. Upper Stomach Pain: She complains of pain on the upper side of her stomach.
d. Breathlessness and Thirst: She frequently experiences sudden bouts of breathlessness and excessive thirst, especially late at night.

We are regularly monitoring her blood pressure, and it is consistently within the normal range.

We have attached the recent sonography reports and her current prescription for your review. I have gone through it and also the attached reports and would like to offer some information.

Firstly, I have noticed two significant points in the sonography report.
1. There are two loops of cord around the neck which calls for fetal wellbeing to be monitored closely.
2. The amount of liquor is lower which needs to be corrected.

Attending to the complaints mentioned by you,

1. Gum bleeding is often found during pregnancy. Use of gentle force, sensitive paste and brush are helpful. Blood work to detect anemia, platelet counts, liver function and protein levels and renal function is needed.

2. If the swelling is not relieved with rest and elevation of the feet even after 12 hours, she needs to see a physician for further evaluation. Mild to moderate swelling in the feet is seen after sitting or standing for some time during this period. Hypertension and hypoproteinemia can accentuate. Labetalol can also cause swelling in the feet.

3. Pain in the stomach can be due to hyper acidity or indigestion. Please specify the exact location of the pain and if it is persistent or temporary. I will be able to comment better.

4. Breathlessness is a symptom that needs further management. It can be caused by the medication though. Hello,

I have gone through your queries and would like to offer my opinions :

1. The presence of two loops of cord around the neck requires added care during delivery like fetal heart rate monitoring, fetal movement check, early intervention if needed etc. During pregnancy, there are no available treatment options at present. However, in the majority of cases, the outcome is uneventful.

2. Increasing the intake of fluids, bed rest, using intravenous fluids if necessary, correcting blood pressure and anemia are some useful measures during pregnancy that help to improve the liquor content. Monitoring can be done by another sonogram at 36 weeks.

3. Gum bleeding is often observed during pregnancy. As I mentioned earlier, use of a soft toothbrush and a sensitive paste along with saline gargles often help. Blood work mentioned earlier is a general precaution since I noticed that you did not mention about any in your reports. Also, it helps to know the clinical condition better in the event of continuous swelling of the feet. A complete hemogram, liver profile and renal profile will help. Interventions needed, if any, depend on the results.

4. As I mentioned earlier, a physiological swelling usually disappears after 8-10 of bed rest with elevation of feet. If it is persistent, please contact your physician for further appraisal. Prolonged standing or sitting should be avoided in pregnancy as this causes swelling in feet.

5. Eating frequent small meals every 2-3 hours and avoiding spicy, oily and fried foods help to ease digestion and prevent heart burn. A glass of cold milk can also help. If the pain is persistent, please inform your consultant.

I would suggest a few measures to help alleviate the pain- using an ortho mattress, sleeping on the side preferably on the left with a soft pillow to support the belly, using a soft pillow under the neck to keep the spine straight as much as possible and a few antenatal exercises as suggested by your consultant.

8. Elevation of the feet, massage during the day, small walks after meals or whenever possible, use of calcium regularly and maintaining the electrolyte balance help to reduce the night cramps.
