Dear Dr. AA The Following Symptoms Have Increased In The

Question: The following symptoms have increased in the last 2 to 3 months:

1. Dizziness / Brain fog with slight pressure on temples
2. Anxiety
3. Weakness in legs
4. Bloating
5. Gas (if I sneeze, I feel better)
6. Face flushed

It is difficult for me to work. Most of the time, anti histamine (kestine 10 mg) relieves the symptoms.

I have IBS, anxiety, Gilbert's syndrome, vitamin D deficiency, and possibly histamine intolerance.

All blood tests (CP, RFT, urine, stool, thyroid, calcium, sugar are clear except LFT with slightly raised biluribin). BP is normally low. But pulse is usually fast.

I am taking
1. Prozac one daily
2. Alp 0.5 mg 1+1+1
3. Levopraid 25mg 1+1
4. Inhibitol one daily
5. Kestine SOS

I have completed 4 capsules of vitamin D (2 lacs) in 1 month.

My psychiatrist has increased Prozac from 1 to 2 daily.

My question are
A. Will increased dose of prozac create other issues?
B. If symptoms subside then upon doctors advice, can Prozac be tapered?
C. Should I request for some other SSRI?
D. Are my medication towards higher side?

Please advise.

Thanks for posting again to me.

During previous communication I suggested for the use of another SSRI such as paroxetine for the better management of your symptoms.

My take on the current questions are as follows
A. Will increased dose of prozac create other issues?
Ans. Most likely increase dose of prozac will help you but there is little chance that it will create more issues. Generally the side effects associated with prozac are either time limited or molecule related. Increased dose does not mean that there will be more side effects. So it can be stated that there is nothing to suggest that it will create more issues.

B. If symptoms subside then upon doctors advice, can Prozac be tapered?
Ans. Yes but after a period of stability. Generally we observe that stability takes months to establish, so the decision to tapering down the dose of medication taken after months of symptom improvement.

C. Should I request for some other SSRI?
Ans. Since recently you changed the dose of medication and you had partial response so far with lower , it will not be wise to change medication at this stage.

D. Are my medication towards higher side?
Ans. Absolutely NO. The dose of prozac varies between 20-80 mg per day and so the current dose can not be considered high. Although you have not mentioned dose of prozac in your description I assume you are using 2 tables of 20 mg prozac.
