Thanks for query ,
Based on the report that you have posted the patient in question seems to have Chronic UTI with Genital Fungal infection .

I shall be happy if you can post me more details to help me in making precise comments
1) What is the age of the patient ?
2) Does she have any vaginal discharge ?

Get her routine urine culture done to find out the organisms causing this infection and antibiotics to which they are sensitive to.

Take broad spectrum antibiotics like Cefixime along with urinary antiseptic like Nitrofurantoin and anti fungal drug like Fluconazole twice daily and urine alkaliser thrice daily.Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

She is 30 years old, married with one 4 years old child
her symtomps are
1.burning sensation while urinating
2.vaginal discharge (clear/no smell)
3. Back pain (feels like muscle strain) Same place as kidneys
