                                            Question : Dear Sir , please find attached ECG provide exact interpretation  (case details - 55 year male Past history of cardiac valvular diseases with co-morbidity – known case of hypertension since 12 to 15 years )

Sinus rhythm with rate and rhythm. It shows left axis deviation and poor R wave progression. No significant ST T abnormalities seen.
So, basically these findings suggest some kind damage to left ventricle of heart or may indicate shifting of heart within chest cavity due to previous valvular heart disease.
So, patient need further evaluation with echo, which will guide in further investigations.
