Question : Dear Urologist (Need a Urologist for this problem),

One of my testicals is larger and firmer than the other so went to do an Ultrasound. Attached you will find the results and report.

In the left testical they found 4 Epidermoids (or Epidermoid Cysts?). I have the following questions:

Is and Epidermoid Cyst in the testicals the same thing as a Spermatocele?

What should I do now? Will they heal by itself or do I have to take action? Surgery?

Can they grow larger? Amd if yes what happens if they become really large? Surgery?

if Surgery is necessary will I loose my testical or can they be removed without removing the testical?

Is there a chance that I can get Testical cancer from those 4 Epidermoids?

Thanks for query .

Based on the facts and reports of your ultrasound Scanning that you have posted I would like to answer your queries as detailed bellow.
1)Epidermoid Cysts are Intra Testicular Cysts where as Spermatocele are in Epididymis and both are different in pathology.

2)Looking at the number of cysts being four and your age It is advisable to get surgery of removal of testicle done (Orchidectomy)

3) These Cysts are prone to undergo malignant changes over a period of time hence it is always safe to get rid of them by removing affected testicle .

4) There are lot of controversy over treatment of Epidermoid cysts. Enucleation of the Epidermoid cyst is advised in young age and children with solitary cysts Since you have 4 cysts and your age being 39 I would suggest to get Testicle removed as there is risk of cyst undergoing malignant changes (Cancer)
