Dear Healthcaremagic I Am A Diabetic Patient And And Discovered

I am a diabetic patient and and discovered my diabetes last year, without any symptom just a random checkupĀ I found this disease...
My Hba1c was 9.6 and morning blood sugar was above 235.. But with a minimum amount of metformin 500 mg daily once, low carb diet and exercise in next three month I bring that down to 6.4% hba1c. And in next 4 month i was at 5.9% (in last august 2020).this was my history now i have some questions. My last(4 month back )test results are attached.
In my last Visit with doctor he told me that even with a low hba1c value, its possible that our Kidney results can give protein in reports. So a checkup is mandatory.. My question is what is the blood sugar value to maintain long term kidney health.?? As of now my age is 34.
Kindly check my test reports which are 4 month old and tell me what is the condition of my kidney as per these reports? . I am just worried because I have a very high blood sugar level at the time of results last year.. And I don't know how much damage has done.
Normal kidney function
Detailed Answer:
Hello, Thank you for using "Ask a doctor" service.
I read your query and reviewed your reports.
The two lab tests related with diabetes and kidney function are creatinine (0.9) and microalbumine in urine (0.8). Both are within normal limits.
As much as you can keep the sugar levels within normal limits, that much you lower the possibility of kidney problems. As of when the lab tests were done, is no kindey damage visible. With diabetes the damage happens very gradually. At some point, even if you do your very best to have a perfect weight and diet, the diabetes will become worse and you will need to start insulin. For some people it takes 10 years, for others 30. Depends on how good you take care of the diet and lifestyle. But because your kidneys can be damaged also very gradually, that is why together with glicemi and hba1c levels, you will need to measure regularly the creatinine and microalbumime in the urine.
Hope this is helpful. Let me know if I can assist you further.
Kind regards, Dr. Klarida Papaqako, General and Family physician.

Insulin resistance
Detailed Answer:
Hello,
Thank you for the follow up question.
Diabetes is a disease which is caused due to insulin resistance. This means that even if insulin levels are high in a persons body, that level is not enough to lower the level of the glucose in the body. So the pancreas, the organ which produces insulin takes the signal of making more insulin until the glucose levels return normal. The insulin resistance comes because either due to obesity, or to genetics or to age the receptors in the body where insulin conects to lower the glucose, are less sensitive. The longer someone has Diabetes, less sensitive those receptors become, the higher becomes the insulin in the body that needs to be produced to keep the glucose levels in normal limits, less possible is for the pancreas cells to make the required insulin amount- until they start gradually stopping working anymore. And this is the stage when a person should move to insulin therapy taken as an injection.
I hope my explanation makes sense. Let me know if you have more questions.
Kind regards,
Dr. Papaqako
