After reviewing your attached reports I would like to tell you that there is a mild increase in serum creatinine levels which can be because Of hypertensive renal disease or an acute injury due to mild cardiac ischemia as ECG is showing T wave inversion in lateral leads.
Kindly get an Echocardiography done and consult a cardiologist for further evaluation and treatment.
