Diagnosed Non Alcoholic Fatty Liver Disease. Has Hypothyroidism, PCOD And Uticaria. Cure?

Question: doctor my wife has NAFLD diagnosed through Usg. Also has PCOD, marginal hypo thyriodsim, uticaria. Are they all connected issues. How to cure these. pls help.

First of all, NAFLD (Non Alcoholic Fatty Liver Disease) is one cause of fatty liver and not related to alcohol intake. It is mostly associated with insulin resistance due to endocrine or metabolic disorder. PCOD (Poly Cystic Ovarian Disease) and hypothyroidism are predisposing factors for the same. Certain medication like aspirin/ steroids etc, soft drinks, obesity, hypertension, genetic predisposition are other associated causes of NAFLD.

Here, I give you an overall treatment plan which can help her to improve present condition:

1) PCOD is an endocrine disorder, associated with insulin resistance. It is a treatable condition. She needs to take low carbohydrate diet, regular moderate to heavy exercise for weight reduction if obesity persists (key role to correct hormonal imbalance), adequate hormonal medication to regularize period & ovulation, insulin sensitiser medicine like metformin etc. These are very much needed to eliminate root cause of NAFLD.

2) Correction of hypothyroidism state is much essential in this context. She needs to take thyroid medication regularly with proper doses. Kindly, consult with her gynecologist for a complete prescription.

3) Other supportive treatment may be needed for NAFLD on the basis of some additional investigation like liver function test (LFT), liver enzymes, lipid profile, blood sugar, blood pressure etc. She needs to avoid soft drinks, high-fructose corn syrup. Ursodeoxycholic acid and lipid-lowering drugs have some benefit.

4) Urticaria is one type of skin rash and may be due to autoimmune disease or allergic reaction. Blood test for differential count (Eosinophil) and Ig E are required to assess the severity of the condition. Kindly consult with a dermatologist for proper clinical examination and suggestive medication or cream/ lotion. She would avoid chemical contacts, allergic substance (like soap/ detergent etc), certain foods (like porn, crab, brinjal, egg etc), hot places/ weather, tight fitting dresses etc.
